Principais causas para o desenvolvimento de lesão renal aguda em pacientes internados em unidade de terapia intensiva: revisão integrativa / Main causes for injury acute kidney development in hospitalized patients in intensive care unit: integrative review

RESUMO

Introdução: a insuficiência renal aguda (IRA) é a perda da função renal iniciada de forma súbita, independentemente da causa ou mecanismo, podendo causar a acumulação de substâncias nitrogenadas, podendo apresentar a diminuição da diurese. Dentre os principais fatores de risco para o desenvolvimento da IRA em uma unidade de terapia intensiva, podem ocorrer eventos isquêmicos, nefrotóxicos, infecciosos, obstrutivos, hipotensão arterial, insuficiências cardiovasculares, hepática e respiratória, neoplasias, gerando um tempo médio de internação superior a sete dias. Objetivo: escrever o conhecimento existente na literatura sobre as principais causas para o desenvolvimento de lesão renal aguda em pacientes internados em unidade de terapia intensiva. Método: realizou-se uma revisão integrativa da literatura, sendo os critérios de inclusão: artigos publicados nos anos de 2005 a 2015, no idioma português, publicados na íntegra, nas bases de dados na Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Electronic Library Online (SCIELO). Resultados e Discussão: incluídos um total de 10 artigos, os principais fatores de risco encontrado foram a sépsis, o choque séptico e as doenças respiratórias e cardiovasculares. Conclusão: o conhecimento de alguns fatores de risco pode contribuir para intervenção e prevenção de disfunções renais e/ou minimizando complicações desses pacientes. Evidencia-se as principais causas para o desenvolvimento de LRA em pacientes internados na UTI como a sépsis, choque séptico, doenças respiratórias e cardiovasculares.

ABSTRACT

Introduction: acute renal failure (ARF) is the loss of sudden onset renal function regardless of cause or mechanism, may cause accumulation of nitrogenous substance and may present decreased urine production. Among the main risk factors for the development of ARI in the ischemic events of the intensive care unit, nephrotoxic, infectious, obstructive, hypotension, cardiovascular, hepatic and respiratory, cancer, generating an average duration of stay of more than seven days. Objective: the objective of this study is to describe the literature knowledge about the main causes of acute renal injury development in patients admitted to intensive care units. He carried out an integrative review of the literature on the main causes for the development of acute kidney injury in the hospital intensive care unit. To access it we have identified descriptors in the site Descriptores de Ciências da Saúde, DECS (decs.bvs.br). The inclusion criteria were: articles published in the years 2005-2015, in Portuguese, that presented the study published in the existing database of the Health Sciences of Latin America and the Caribbean (LILACS) and the Scientific Electronic Library online (SCIELO ). Results and Discussion: it was included a total of 11 articles, 6 extracted from the SCIELO database and 5 extracted from the LILACS database. The main risk factors were sepsis, septic shock and respiratory and cardiovascular disease. Conclusion: concluding that knowledge of some risk factors may contribute to the intervention and prevention of renal and / or minimizing complications in these patients. It is evidenced that the main causes for the development of AKI in patients hospitalized in the ICU such as sepsis, septic shock, respiratory and cardiovascular diseases.
