Representações do enfermeiro sobre infecção em pacientes submetidos ao transplante de células-tronco hematopoiéticas / Nurse's representations about infection in hematopoietic stem cells transplant

RESUMO

A infecção em pacientes submetidos a Transplante de células-tronco hematopoiéticas é uma das principais complicações e relevante causa de mortalidade nessa população. Esta pesquisa objetivou identificar as representações dos enfermeiros sobre infecção em pacientes submetidos ao Transplante de Células-tronco Hematopoiéticas. Trata-se de uma pesquisa qualitativa realizada no serviço de Transplante de Células-tronco Hematopoiéticas do Hospital de Clínicas da Universidade Federal do Paraná com 36 enfermeiros assistenciais do transplante, no período de março a maio de 2010. Os dados foram coletados por meio de entrevista semiestruturada e analisados de acordo com análise de conteúdo. Como resultado emergiu duas categorias infecção e a possibilidade de finitude humana e infecção e culpabilidade. Os enfermeiros relacionaram a infecção com a imunodepressão do paciente que pode culminar no óbito; esta representação se ancora no dia a dia desses profissionais, que consideram o condicionamento requerido para o tratamento como preditor de agravos, na segunda categoria, apreendeu-se nas falas, que este vivencia em seu cotidiano a culpabilidade frente à ocorrência de infecção, considerando que as medidas preventivas falharam. O enfermeiro sente-se responsável pela educação em saúde da equipe de enfermagem, multidisciplinar, paciente e cuidador. Para os enfermeiros a infecção em pacientes submetidos ao transplante é um problema cotidiano e sua ocorrência é relacionada negativamente à assistência prestada. O desafio para os enfermeiros assistenciais é centrado em medidas preventivas de infecção. Desse odo, utiliza o conhecimento técnico-científico em seu cotidiano com vistas a diminuir esses agravos.

ABSTRACT

The infection in patients undergoing transplant hematopoietic stem cells is a major complication of this therapy and significant cause of mortality in this population; the professionals working in this specialty live daily with a series of complications. The objective of this research is to identify the representations of nurses about infection in patients undergoing transplantation of hematopoietic stem cells. This is a qualitative research conducted in Transplantation of Hematopoietic Stem Cell service's at Hospital de Clínicas of Universidade Federal do Paraná, of 36 clinical nurses from the transplanting, in the period from March to May 2010. Data were collected through semi-structured interviews and analyzed according to content analysis. The data allowed to understand two categories infection and the possibility of human finitude and Infection and culpability. The nurses related to infection with patient's immunosuppression that may result in death, this representation is grounded in dayto- day lives of these professionals, who consider the conditioning required for treatment as a predictor of injuries. Referring nurse as a vector for infections seized on the speech, was observed on this experience that in their daily a culpability felling its present against the occurrence of infection, whereas preventive measures have failed, because he feels responsible for health education of the nursing staff, multidisciplinary, patient and caregiver. For nurses, infection in transplant patients is an everyday problem and its occurrence is negatively related to health care. The challenge for clinical nurses is focused mainly on preventive measures of infection in this way, using the echnical-scientific knowledge in their daily lives in order to reduce these infections.
