Validez, seguridad utilidad de, y exactitud circunferencia de cintura para diagnosticar obesidad en población adulta de establecimientos / Validity, security utility of waist circumference to, and exactitud diagnostic obesity in adult people in health facilities of Chiclayo

RESUMEN

Objetivos: Precisar cuál criterio internacional (ATP III, IDF Europeo-Asiático ó LASO) según circunferencia de la cintura (CC) ofrece mayor validez, seguridad, utilidad y exactitud para diagnosticar obesidad en población adulta de Establecimientos de Salud de Chiclayo. Material y Métodos. Estudio Descriptivo Transversal, Tipo Prueba de Pruebas. La muestra fue seleccionada de tres Hospitales (Naylamp, Almanzor Aguinaga Asenjo y Las Mercedes) y tres Centros de Salud (José Olaya, Cerropón y Cruz de la Esperanza), captándose 422 personas. Se realizaron mediciones de peso, talla y CC y comparaciones con los valores a partir de los cuales se considera obesidad en los criterios de ATPIII; IDF Europeo; IDF Asiático y LASO. El Gold Standard fue el Índice de Masa Corporal (IMC). Se obtuvo sensibilidad, especificidad, valores predictivos, coeficientes de probabilidad, por cada criterio según sexo. Utilizamos para el análisis de datos la calculadora estadística Epidat 3.1. Resultados. ATPIII presentó mayor especificidad 91,38%, valor predictivo positivo 71,43 % y coeficiente de probabilidad positivo 8,79 en varones. LASO presentó mayor especificidad 56,67%, valor predictivo positivo 40,13% y menor coeficiente de probabilidad negativo en mujeres. Conclusiones.El estándar internacional de mayor validez, seguridad y utilidad en varones es ATPIII y en mujeres, LASO.(AU)

ABSTRACT

Introduction: To specify which international criteria (ATP III, IDF or European-Asian or LASO) according to the circumference of the waist (WC) is most valid, safe and useful to diagnose obesity in the adult population of Chiclayo Health centers. Material and Methods: Transversal Descriptive Study, test of tests type. The sample was selected from three hospitals (Naylamp, Almanzor Aguinaga Asenjo and Las Mercedes Hospitals) and three health centers (José Olaya, Cerropón and Cruz de la Esperanza), recruiting 422 people. Weight, height, WC measurements and comparisons were made with established values from which obesity is considered according to ATPIII, European IDF, Asian IDF and LASO criteria. The Gold Standard was the Body Mass Index (BMI). Sensitivity, specificity, predictive values and probability coefficients were obtained, for each criterion according to the sex. For the data analysis; we used the Epidat calculator 3.1.Results: ATPIII had a higher specificity of 91.38%, positive predictive value of 71.43% and positive probability coefficient of 8.79 for males. LASO had a higher specificity of 56.67%, positive predictive value of 40.13% and lower negative probability coefficient for females. Conclusions: The most international valid, safe and useful standard for males is ATPIII and for females is LASO.(AU)
