La infección respiratoria aguda no neumónica en menores de 5 años y las expectativas maternas en la atención clínica / Maternal expectations for the treatment of non-pneumonic acute respiratory infection in children under five years

ABSTRACT

Objective: To identify maternal expectations for the treatment of non-pneumonic severe acute respiratory infection (SARI) in children under five years of age provided by a hospital in an indigenous area in southwestern Colombia, 2012. Method: A PAHO-WHO Community Focused Ethnographic Study of SARI was conducted in an indigenous population, including seven health workers from the village hospital and 50 mothers seeking medical attention for their children under five presenting with SARI. Mothers were interviewed to determine their expectations regarding medical care. Health workers were also interviewed to determine their perspectives on maternal knowledge and healthcare-seeking behavior. Information was analyzed by categories through descriptive matrices, which were grouped by themes. Results: Interviews demonstrated that most mothers were not able to clearly identify different types of medications, nor the difference between antibiotics and medications designed to treat symptoms. Most mothers were also unable to identify the effect and mode of action of the relevant medications. The main concern expressed by the mothers was to relieve the symptoms of SARI. Most health workers reported a delay in mothers seeking medical help for their children. Conclusion: Care of acute respiratory infections in the Western conception of the biomedical model is foreign to most indigenous mothers. This, together with the lack of health workers' perceptions and understandings of maternal knowledge, helps prevent improvement in the morbidity and mortality of SARI

RESUMEN

Objetivo: Identificar las expectativas maternas del tratamiento de infección respiratoria aguda grave no neumónica (IRAG) en niños menores de 5 años brindado por un hospital de una zona indígena del suroccidente colombiano, 2012. Método: Estudio comunitario etnográfico focalizado de IRAG de la OPS-OMS, realizado en un pueblo indígena, en siete trabajadores de la salud del hospital y 50 madres que buscaron la atención médica para sus hijos menores de 5 años con IRAG. Se aplicó una entrevista a las madres para conocer sus expectativas en la atención médica; a los trabajadores de salud se les aplicó una entrevista, para conocer la percepción sobre el reconocimiento materno y la búsqueda de atención médica; la información se analizó por categorías a través de matrices descriptivas, que se agruparon por similitud conceptual. Resultados: Se pone de manifiesto que la mayoría de las madres no identifican claramente los diferentes tipos de medicinas; no encuentran diferencias entre antibióticos y medicamentos sintomáticos; no pueden identificar el efecto y la forma de acción del medicamento; la principal preocupación de las madres es curar los síntomas de la IRA; la mayoría de los agentes de salud consideran que hay tardanza en la búsqueda de ayuda médica. Conclusión: La atención de infecciones respiratorias agudas bajo la concepción occidental del modelo biomédico resulta ajeno y desconocido para las madres indígenas, que junto al desconocimiento de agentes de salud de las percepciones y entendimientos maternos indígenas, contribuyen a mantener la morbimortalidad de las IRAG
