RESUMEN

Eres el médico residente a cargo de Emergencias en el Hospital Vozandes del Oriente, en Shell, Pastaza. Durante la noche llega un agricultor que fue víctima de una mordedura de serpiente venenosa hace una hora mientras trabajaba en actividades agrícolas. Entre otras cosas te preocupa el riesgo de hemetotoxicidad y coagulopatía, especialmente si la mordedura fue de un crótalo del género Bothrops, frecuente en la zona, En el arsenal de fármacos disponibles, entre antibióticos y analgésicos soluciones parenterales y demás, encuentras algunos viales de suero antiofídico poliespecífico, varios Ecuatorianos, preparados por el Instituto Izquieta Pérez en Guayaquil, otros Colombianos del Instituto Nacional de Salud y otros más del Instituto Butantan de Brasil. ¿Cual escoger? Inmediatamente de iniciar con medidas generales, vienen a tu mente las prácticas que realizaste en uno de varios talleres de Medicina basada en evidencia (MBE) que cursaste durante tu formación de pregrado, especialmente lo que aprendiste del ya famoso libro de David Sackett.

ABSTRACT

You are the resident doctor in charge of Emergencies at the Hospital Vozandes del Oriente, in Shell, Pastaza. During the night a farmer arrives who was the victim of a poisonous snake bite an hour ago while working in agricultural activities.
