RESUMEN
INTRODUCCIÓN. La ocurrencia de un primer ictus isquémico conlleva morbilidad impor-tante y riesgo de recurrencia de un nuevo evento en donde tienen un rol preponderante los factores de riesgo. OBJETIVOS. Demostrar que existe mayor exposición a factores de riesgo en los ictus recurrentes que en los únicos; identificar el grado de asociación de cada uno de los factores de riesgo con ictus recurrente. MATERIALES Y MÉTODOS. Estudio de casos controles, población 1 011 con una muestra de 509 pacientes, obtenidos de los ingresos a la Unidad Técnica de Neurología del Hospital de Especialidades Carlos Marín, de enero 2009 - diciembre 2015, con diagnóstico de ictus isquémico primero/recurrente, los datos obtenidos del sistema informático AS400 fueron edad, sexo, número/tipo de factores de riesgo hipertensión arterial, diabetes mellitus, fibrilación auricular, cardiopatía isquémica, dislipidemia, fumador, insuficiencia cardíaca congestiva. Se estimó la fuerza de asociación a través del OR y su IC al 95%. RESULTADOS. Edad promedio 71,5 años, el 57,4% (292; 509); correspondieron al género masculino. El análisis de variables cua-litativas mediante Chi cuadrado y la estimación de riesgo según tablas de contingencia demostró asociación entre exposición a factores de riesgo tradicionales y recurrencia de ictus con un OR 1,30, IC 95% (0,69-2,45) p = 0,41; el análisis multivariado mediante re-gresión logística para cada factor de riesgo y recurrencia de ictus demostró asociación estadística hipertensión arterial OR 2,05; diabetes mellitus OR 1,52; fibrilación auricular 1,83. CONCLUSIÓN. Los datos de este estudio demostraron asociación de riesgo para recurrencia de ictus con tres factores de riesgo modificables tradicionales.
ABSTRACT
INTRODUCTION. The occurrence of a first ischemic stroke involves significant morbidity and risk of recurrence of a new event where risk factors have a preponderant role. OB-JECTIVES. Demonstrate that there is greater exposure to risk factors in recurrent strokes than in single strokes; identify the degree of association of each of the risk factors with recurrent stroke. MATERIALS AND METHODS. Case-control study, population 1 011 with a sample of 509 patients, obtained from admission to the Neurology Technical Unit of the Carlos Marín Specialty Hospital, from january 2009 to december 2015, with a diagnosis of ischemic stroke first / recurrent, data obtained from the AS400 computer system were age, sex, number / type of risk factors arterial hypertension, diabetes mellitus, atrial fibrilla-tion, ischemic heart disease, dyslipidemia, smoker, congestive heart failure. The strength of association was estimated through the OR and its 95% CI. RESULTS. Average age 71,5 years, 57,4% (292; 509); corresponded to the male gender. Chi-square analysis of qualitative variables and risk estimation using contingency tables showed an association between exposure to traditional modifiable risk factors and stroke recurrence with an OR 1,30 95% CI (0,69-2,45) p=0,41; multivariate analysis using logistic regression for each risk factor and recurrent stroke showed statistical association arterial hypertension OR 2,05; diabetes mellitus OR 1,52; atrial fibrillation 1,83. CONCLUSION. Data from this study demonstrated a risk association for recurrent stroke with three traditional modifiable risk factors.
