Validación de la Guía general para diagnosticar barreras que afectan la adherencia terapéutica en hipertensos / Validation of the General Guideline for diagnosing barriers affecting therapeutic adherence in hypertensive people

RESUMEN

RESUMEN Introducción: los pacientes hipertensos muestran dificultades en la adherencia al tratamiento, lo que justifica la construcción y validación de instrumentos para identificar las barreras de este comportamiento. Objetivo: validar la Guía general orientadora para diagnosticar barreras que afectan la adherencia terapéutica en pacientes hipertensos, su contenido y aplicación en la muestra seleccionada. Métodos: se aplicaron métodos teóricos de integración para dirigir el enfoque del cuestionario propuesto, el cual se elaboró para ser utilizado en grupos focales. La validación del contenido se realizó por valoración de expertos. El instrumento fue aplicado en una muestra de 100 hipertensos esenciales, 142 proveedores y 12 directivos, de las tres áreas de salud del municipio Remedios. Resultados: se integraron las cinco dimensiones como determinantes de la adherencia terapéutica, con tres dimensiones de la determinación social de la salud. Esto permitió dar una estructura óptima al cuestionario nombrado Guía general orientadora. Se obtuvo un diagnóstico dentro del escenario de acción de la salud en el territorio y el análisis se enfocó en los actores sociales. El coeficiente de concordancia entre expertos fue de 0,82, para los criterios razonable, comprensible y sensible a variación. Se demostró que las categorías incluidas en el instrumento son apropiadas. Se obtuvieron las barreras que afectan la adherencia terapéutica en los hipertensos de las áreas estudiadas. Conclusiones: los resultados de la prueba de validación del contenido que realizaron los expertos, así como los resultados del diagnóstico, evidenciaron que la Guía general es un instrumento útil.

ABSTRACT

ABSTRACT Introduction: hypertensive patients show difficulties to treatment adherence, which justifies the construction and validation of instruments to identify the barriers to this manifestation. Objective: to validate the General Guideline to diagnose barriers affecting therapeutic adherence in hypertensive patients, its content and application in the selected sample. Methods: theoretical integration methods were applied to guide the proposed questionnaire approach, which was developed to be used in focus groups. Content validation was done by expert assessment. The instrument was applied to a sample of 100 essential hypertensive people, 142 providers and 12 managers, from the three health areas of Remedios municipality. Results: the five dimensions were integrated as determinants of therapeutic adherence, with three dimensions of the social determination of health. This allowed giving an optimal structure to the questionnaire named General Guideline. A diagnosis was obtained within the scenario of health action in the territory and the analysis was focused on the social actors. The coefficient of agreement between experts was 0.82, for the criteria reasonable, comprehensible and sensitive to variation. The categories included in the instrument were shown to be appropriate. Barriers affecting therapeutic adherence in hypertensive people in the studied areas were obtained. Conclusions: the results of the content validation test carried out by the experts, as well as the results of the diagnosis, showed that the General Guide is a useful tool.
