Resumen Se presenta el caso de un paciente joven, inmunocompetente y sin factores de riesgo, con afección compatible con síndrome mononucleósico (adenopatías cervicales, fiebre, malestar general), que se caracterizó por persistencia de síntomas, a pesar de múltiples tratamientos con antibióticos. Durante su abordaje se descartó infección por las causas más habituales de fiebre e infección por Epstein-Barr, por lo cual se realizaron pruebas serológicas donde se corroboró una etiología que es poco habitual que se presente como mononucleosis infecciosa. Al identificar al agente infeccioso y debido a que se asoció con alteraciones hematológicas, organomegalias y miositis durante su evolución, el paciente fue candidato a tratamiento antibiótico, con notable mejoría al iniciarlo, lo que lo llevó a su recuperación total.
ABSTRACT
Abstract We present the case of a young patient, immunocompetent and without risk factors, with a condition compatible with mononucleosis syndrome (cervical lymphadenopathy, fever, malaise), which was characterized by persistent symptoms, despite multiple antibiotic treatments. During its approach infection was ruled out due to the most common causes of fever and Epstein-Barr infection, for which serological tests were performed where it was corroborated an etiology that is unusual that is presented as infectious mononucleosis. By identifying the infectious agent and because it was associated with haematological alterations, organomegalias and myositis during its evolution, the patient was a candidate for antibiotic treatment, with notable improvement when he started it, which led to his total recovery.
