Compreendendo o processo de sofrimento no trabalho do Agente Comunitário de Saúde no Programa de Saúde da Família / Understanding the suffering process at work of a Community Health Agent in a Family Health Program

RESUMO

A pesquisa, de natureza qualitativa, teve como núcleo de interesse investigativo as representações e as significações que um grupo de Agentes Comunitários de Saúde (ACSs) possuem a respeito das vulnerabilidades para o sofrimento no trabalho a que estão expostos, assim como as próprias manifestações deste sofrimento ao desempenharem suas ações profissionais no Programa de Saúde da Família (PSF). Os dados foram coletados por meio de entrevista semiestruturada com um grupo de 12 ACSs de uma Unidade Básica de Saúde na cidade de São Paulo , que tem exclusivamente como modelo assistencial o PSF. O material obtido foi analisado à luz do referencial teórico-metodológico da hermenêutica, que atende perfeitamente à finalidade de utilizá-la como instrumental interpretativo da linguagem e de identificação do significado do discurso. A base teórica da psicodinâmica do trabalho também foi empregada na análise do material obtido, com vistas a sustentar teoricamente os argumentos referentes especificamente ao sofrimento no trabalho. As questões, que se concentraram no significado de ser Agente Comunitário de Saúde e na percepção da organização do trabalho, conduziram à construção de grandes núcleos temáticos: a) Análise de vulnerabilidades no trabalho, configuradas a partir da significação que o ACS confere à sua profissão; b) Análise da compreensão do sofrimento referido, configurada a partir da significação que o ACS confere à sua profissão; c) Análise de vulnerabilidades no trabalho, configuradas a partir da representação que o ACS constrói em torno da organização do trabalho; d) Análise da compreensão do sofrimento referido, configurada a partir da representação que o ACS constrói em torno da organização do trabalho. Os elementos enumerados validam a existência de uma importante vulnerabilidade ao sofrimento no trabalho, em razão da tendência do ACS em assumir também as deficiências inerentes ao Programa, ) ora por falta de preparo e suporte técnico continuado, ora por dificuldades de acesso da população que traduzem as limitações próprias da rede do Sistema Único de Saúde (SUS) local, face ao conjunto de inovações a que o PSF propõe e não conclui, tendo este profissional pouca perspectiva de rearranjo dos ingredientes constitutivos da organização do trabalho, já que depende de fatores alheios a seu espectro de alcance. No entanto, observou-se, concomitantemente a existência de mecanismos intrapsíquicos no ACS, relacionados à estrutura da personalidade, que amplificam, na instância do trabalho, a vulnerabilidade e a respectiva vivência do sofrimento, sobretudo, apoiadas na construção da ideação idealizada da prática profissional.

ABSTRACT

This qualitative research aimed at investigating the representations and significations that a group of Community Health Agents (CHAs) has concerning the vulnerabilities related to the suffering at work they are exposed to, as well as the manifestations of this suffering as they perform their actions at the Family Health Program (FHP). The data were collected from a semi-structured interview carried out by a group of 12 CHAs from a Basic Health Center in the city of São Paulo which has FHP as its exclusive assistential model. The data were analyzed based on the hermeneutic theory methodology that is a perfect instrument for interpreting language and identifying the discourse meaning. Work psychodynamic theory baseline was also used to analyze the data in order to theoretically support the arguments referring to suffering at work. The questions related to the meaning of being a Community Health Agent and to the perception concerning work organization, led to the construction comprehensive thematic centers: a) The analysis of vulnerability at work from the meaning the CHA gives to their profession; b) The analysis of how suffering is understood from the meaning the CHA gives to their profession; c) The analysis of vulnerability at work from the representations that the CHA constructs around work organization; d) The analysis of how suffering is understood from the representations that the CHA constructs around work organization. The ranked elements validate the existence of an important vulnerability of the suffering at work due to the tendency of the CHA to assume the deficiencies inherent to the Program one moment due to the lack of preparation and continued technical support, next due to the difficulties of accessibility faced by the population what translates the local SUS (Unique Health Service System) owns limitations, in the face of a set of innovations that FHP intends to do and it does not. This professional has little perspective of rearranging the constitutive ingredients of work organization, since they depend on factors beyond their grasp. However, it was also observed besides the existence of the intra-psychological mechanisms in the CHA, related to the personality structure that increases the vulnerability and the respective suffering faced at work, mainly from the construction of the idealized ideation of the professional practice.
