Construção e validação de instrumentos para a sistematização da assistência de enfermagem em uma clínica de enfermagem em estomaterapia / The development and validation of instruments for the systematization of nursing care in a stomatherapy nursing clinic

RESUMO

O objeto de estudo trata da elaboração e validação de instrumentos para desenvolvimento da sistematização da assistência de enfermagem (SAE) em uma clínica de enfermagem em estomaterapia, bem como da criação de um software e de um aplicativo móvel para facilitar o processo de trabalho neste cenário assistencial. No contexto do atendimento ambulatorial, caracterizado pela necessidade de cura de desordens biopsicossociais de pessoas e famílias, faz-se necessário assegurar um cuidado de qualidade e respaldado por uma metodologia assistencial de enfermagem a fim de nortear as ações neste âmbito do serviço de saúde. Especialmente, citam-se os pacientes assistidos em situações de estomaterapia, de modo a oferecer um cuidado individualizado, de acordo com as necessidades de cada pessoa com estomias, feridas agudas e crônicas, fístulas, drenos, cateteres e incontinências anal e urinária. O objetivo geral deste estudo é elaborar e validar os instrumentos que compõem a SAE em uma clínica de enfermagem em estomaterapia. Apresentam-se como objetivos específicos 1) elaborar os instrumentos para o desenvolvimento da SAE aplicados a pessoas em situação de estomaterapia; 2) testar, por meio de validação de juízes, a aplicabilidade dos instrumentos produzidos que comporão a SAE no contexto da estomaterapia; 3) produzir um software e um aplicativo móvel a fim de socializar os instrumentos para desenvolvimento da SAE à pessoas com feridas, estomas e incontinências. O referencial teórico fundamentou-se nas concepções de Gordon, North-American Nursing Diagnosis Association (NANDA) e no conhecimento da estomaterapia, e as bases conceituais deste estudo foram os cuidados de enfermagem a pessoas com feridas, estomas e incontinência. Trata-se de um estudo metodológico, o qual é definido como investigação de métodos de obtenção, organização e análise de dados, bem como elaboração, validação e avaliação dos instrumentos e técnicas de pesquisa. O estudo foi desenvolvido em quatro etapas, na primeira (1), foi realizada a construção dos instrumentos, trata-se de três roteiros estruturados de sistematização das consultas, do tipo checklist composto por 100 itens, divididos em seis blocos (identificação, exame físico, diagnóstico de enfermagem, evolução, intervenções e resultados esperados), que correspondem aos passos da sistematização da assistência em feridas, estomas e incontinências. Na segunda etapa (2), foi realizada a identificação, seleção e convite aos especialistas a fim de efetuar o julgamento dos itens dos instrumentos; e a terceira etapa (3) correspondeu à validação de conteúdo dos instrumentos com a verificação do nível de concordância entre os juízes e análise semântica para posterior confecção do software. A quarta etapa (4) caracterizou-se na construção do software e aplicativo móvel por meio da contribuição de técnicos de informática (TI), que elaboraram o layout do software e do aplicativo a partir do que foi consolidado no desenvolvimento dos instrumentos da SAE. A execução deste estudo foi relevante para a área de estomaterapia, pois agrega num só aplicativo todas as situações de cuidados a pessoas com feridas, estomias e incontinências, facilitando o processo de trabalho neste contexto assistencial.

ABSTRACT

The study object deals with the elaboration and validation of instruments for the development of the systematization of nursing care in a stomatherapy nursing clinic, as well as the creation of a software and a mobile application to facilitate the work process in this care setting. In the context of outpatient care, characterized by the need to cure biopsychosocial disorders of individuals and families, it is necessary to ensure a quality care, supported by a nursing care methodology in order to guide the actions in this scope of the health service. In particular, patients assisted in stomatherapy situations can be mentioned, so as to offer them an individualized care according to the needs of each person with ostomies, acute and chronic wounds, fistulas, drains, catheters and anal and urinary incontinence. The general objective of this study is to elaborate and validate the instruments that make up the systematization of nursing care in a stomatherapy nursing clinic. The specific objectives are 1) to elaborate the instruments for the development of the systematization of nursing care applied to people in a stomatherapy situation; 2) to test, through the validation of judges, the applicability of the instruments produced that will compose the systematization of nursing care in the context of stomatherapy; 3) to produce a software and a mobile application to socialize the instruments for the development of the systematization of nursing care to people with wounds, ostomies and incontinence. The theoretical framework was based on the conceptions of Gordon, the North-American Nursing Diagnosis Association (NANDA), and the on knowledge of stomatherapy, and the conceptual bases of this study were the nursing care for people with wounds, ostomies and incontinence. It is a methodological study, which is defined as the investigation of methods of obtaining, organizing and analyzing data, as well as the elaboration, validation and evaluation of the instruments and research techniques. The study was developed in four stages. On the first stage (1), the construction of the instruments was carried out, which consisted of three structured scripts for the systematization of the consultations, in a checklist format, composed of 100 items divided into six blocks (identification, physical examination, nursing diagnosis, evolution, interventions and expected results), which correspond to the steps of the systematization of care for wounds, ostomies and incontinence. On the second stage (2), the identification, selection and invitation to the specialists was carried out in order to have the items of the instruments evaluated by them; and the third stage (3) corresponded to the validation of the instruments content with the verification of the level of concordance between the judges and semantic analysis for later confection of the software. The fourth stage (4) was characterized by the construction of the software and mobile application with the contribution of computer technicians, who elaborated the sofware and the application layout from what had been consolidated in the development of systematization of nursing care instruments. The execution of this study was relevant to the stomatherapy area, since it combines in a single application all care situations for people with wounds, ostomies and incontinence, facilitating the work process in this care context.
