Resumen OBJETIVO:
Comparar las complicaciones en embarazadas con diagnóstico positivo de COVID-19 aguda y pasada atendidas en dos establecimientos de salud de Lima Metropolitana, Perú. MATERIALES YMÉTODOS:
Estudio descriptivo, analítico y retrospectivo efectuado en pacientes embarazadas atendidas en dos centros materno infantiles de Lima Metropolitana de junio a diciembre de 2020 en quienes se aplicó la prueba de anticuerpos para COVID-19. Los datos recolectados se trasladaron a una matriz del programa SPSS 25 y se analizaron con prueba exacta de Fisher, cálculo de razón de momios e IC95%.RESULTADOS:
Se analizaron 177 embarazadas con reporte positivo a una prueba rápida One Step test for Novel Coronavirus (2019-nCov) IgM/IgG Antibody (Coloidal Gold). La mayoría tuvo infección pasada, con solo IgG (50.0%), seguidas de infección aguda con IgM e IgG (43.2%). Solo el 6.8% tuvo infección en su etapa inicial, identificada solo con IgM. La única complicación con diferencia significativa entre las embarazadas con COVID-19 positiva, con infección aguda e infección pasada, fue la ruptura prematura de membranas, con un valor de p = 0.019 con la prueba exacta de Fisher. La complicación materna más frecuente fue la ruptura prematura de membranas con 16.4% y la culminación del parto fue mediante cesárea en el 27.7%. Solo se encontró una asociación significativa entre la infección aguda y la ruptura prematura de membranas (p = 0.019; OR = 2.563 IC95% 1.115-5.892).CONCLUSIÓN:
La ruptura prematura de membranas fue la única complicación materna que tuvo asociación significativa con la infección aguda por Covid-19 durante el embarazo.ABSTRACT
Abstract OBJECTIVE:
To compare maternal complications in pregnant women with a positive diagnosis of acute and past COVID-19 attended in two health facilities in Metropolitan Lima, Peru. MATERIALS ANDMETHODS:
Descriptive, analytical and retrospective study carried out in pregnant patients attended in two maternal and child centers of Metropolitan Lima from June to December 2020 in whom the COVID-19 antibody test was applied. The data collected were transferred to a matrix of the SPSS 25 program and analyzed with Fisher's exact test, calculation of odds ratio and 95%CI.RESULTS:
177 pregnant women with a positive report to a rapid One Step test for Novel Coronavirus (2019-nCov) IgM/IgG Antibody (Colloidal Gold) were analyzed. Most had past infection with IgG alone (50.0%), followed by acute infection with IgM and IgG (43.2%). Only 6.8% had early-stage infection, identified only with IgM. The only complication with significant difference between COVID-19 positive pregnant women with acute infection and past infection was premature rupture of membranes, with a value of p = 0.019 with Fisher's exact test. The most frequent maternal complication was premature rupture of membranes with 16.4%, and delivery was terminated by cesarean section in 27.7%. Only a significant association was found between acute infection and premature rupture of membranes (p = 0.019; OR = 2.563 CI95% 1.115-5.892).CONCLUSION:
Premature rupture of membranes was the only maternal complication that had a significant association with acute COVID-19 infection during pregnancy.
