Atenção primaria à saúde no âmbito do SUS: mudanças institucionais em curso no cenário pós-2016 / Primary health care within the scope of SUS: ongoing institutional changes in the post-2016 scenario

RESUMO

A austeridade fiscal imposta pelo Estado Brasileiro às políticas sociais a partir da Emenda Constitucional nº 95/2016 direciona um conjunto de mudanças organizacionais no interior da Atenção Primária à Saúde no âmbito do Sistema Único de Saúde. A fim de respeitar o novo regime fiscal, a desestruturação das políticas públicas de saúde no campo da atenção primária se evidencia em alguns fatos aqui selecionados a nova organização da Política Nacional de Atenção Básica; a desconstrução do Programa Mais Médicos; a criação do Programa Previne Brasil; e as propostas privatistas do Programa Médicos pelo Brasil e da Adaps. O estudo percorre as mudanças normativas na organização da Atenção Primária, explorando o contexto político pós-2016 para sua compreensão. Tem na matriz de Mahoney e Thelen o caminho para reconhecer e tipificar as mudanças das regras institucionais em curso, ainda que algumas não tenham sido efetivamente implantadas, mesmo que normatizadas. Reconhece-se que as mudanças vêm ocorrendo de forma gradual, sem a ruptura completa de uma regra por outra, mas por desvio, conversão e sobreposição entre as regras já existentes, com consequente enfraquecimento da política pública e evidências de uma corrente privatista em seu interior. E faz-se um alerta as mudanças, ainda que graduais, podem produzir transformações de maior dimensão e de difícil reversão ao longo do tempo.

ABSTRACT

The fiscal austerity imposed by the Brazilian State on social policies from Constitutional Amendment No. 95/2016 directs a set of organizational changes within the Primary Health Care within the scope of the Unified Health System. In order to respect the new fiscal regime, the disruption of public health policies in the field of primary care is evidenced in some facts selected here the new organization of the National Primary Care Policy; the deconstruction of the Mais Médicos Program; the creation of the Prevent Brazil Program; and the privatist proposals of the Doctors for Brazil Program and Adaps. The study covers the normative changes in the organization of Primary Care, exploring the post-2016 political context for its understanding. Mahoney and Thelen's matrix is the way to recognize and typify the changes in institutional rules in progress, even if some have not been effectively implemented, even if standardized. It is recognized that changes have been occurringgradually, without the complete rupture of one rule by another, but by deviation, conversion and overlapping between existing rules, with the consequent weakening of public policy and evidence of a privatist current within it. And there is a warning changes, even if gradual, can produce larger transformations that are difficult to reverse over time.
