Manejo antibiótico empírico y profiláctico de la apendicitis aguda no complicada / Empirical and prophylactic antibiotic management of uncomplicated acute appendicitis

RESUMEN
La importancia de la antibioticoterapia en el manejo general de las infecciones intraabdominales ha sido difícil de objetivar y sólo hasta fechas recientes se ha probado su papel esencial en la curación sin complicaciones de este tipo de infecciones. Métodos: Estudio prospectivo y aleatorio en el que se incluyeron 129 pacientes con promedio de edad entre 19 y 60 años que ingresaron al servicio de Cirugía General Dr. José León Tapia Contreras del Hospital Dr. Luis Razetti, entre el 2º de enero de 2016 y el 30 de marzo de 2016, en quienes se efectuó diagnóstico pre-quirúrgico de apendicitis aguda no complicada, y fueron intervenidos quirúrgicamente y tratados con terapias antibióticas empíricas. Resultados: Para demostrar la significancia de la profilaxis antibiótica empírica utilizada en nuestro estudio, la mitad de las infecciones del sitio quirúrgico se presentaron en un grupo de 22 pacientes que recibieron antibioticoterapia combinada con (aztreonam y metronidazol) y la otra mitad se presentaron en la mayoría de la muestra tratada solo con aztreonam, siendo este grupo los de mayor porcentaje con un 75,6 del total de la muestra tratada. Conclusión: Se plantea determinar la eficacia del aztreonam + metronidazol preoperatorio en pacientes con apendicitis aguda no complicada, limitar el uso innecesario de antibiótico para detener el daño colateral, analizar el apego o no de los médicos tratantes y pacientes tratados con el manejo antibiótico empírico planteado(AU)
ABSTRACT
The importance of antibiotic treatment in the general management of intra-abdominal infections has been difficult to objectify and only until recently has tested its essential role in healing without complications from infections. Methods: Prospective and random study in which 129 patients with average age between 19 and 60 years who entered the service of Surgery General Dr. José León Tapia Contreras of the Hospital Dr. Luis Razetti, between of January 2, 2016 and March 30, 2016, in which pre-surgical diagnosis of uncomplicated acute appendicitis, and were operated and treated with empirical antibiotic therapies. Results: To demonstrate the significance of antibiotic prophylaxis empirical used in our study, half of the surgical site infections arose in a group of 22 patients that they received antibiotic therapy combined with (aztreonam and metronidazole) and the other half is presented in most of the treated sample only with aztreonam, being this group the highest percentage with a 75.6 total of the treated sample. Conclusion: Arises to determine the efficacy of aztreonam + preoperative metronidazole in patients with acute appendicitis not complicated, limiting the unnecessary use of antibiotic to stop the collateral damage, analyze the attachment or not of the treating physicians and patients treated with empiric antibiotic management raised(AU)
