Apendicite aguda perfurativa por infestação de Trichocephalus trichiurus / Perfurative acute appendicitis with infestation of Trichocephalus trichiurus

RESUMO

É apresentado um caso de apendicite aguda perfurativa por Trichocephalus trichiurus com infestação maciça por esse helminto, demonstrando-se microscopicamente o caminho fistuloso na parede apendicular, dando passagem ao conteúdo - material fecaloide, ovos e vermes adultos - para a cavidade abdominal. Chama-se a atenção para a ausência de um exsudato inflamatório do tipo supurativo, encontrado nas apendicites agudas; a discreta reação inflamatória e a dissociação dos tecidos falam a favor de um fermento lítico produzido pelos vermes. É possível que a presença de uma hepatite infecciosa, no presente caso, diminuindo a resistência do paciente, tenha contribuído para facilitar a perfuração apendicular.(AU)

ABSTRACT

A case of perforative acute appendicitis with a massive infestation of Trichccephalue trichiurus is presented. It was demonstrated microscopically the fistulous way in the appendiccal wall permitting the entrance of the fecal matter from the appendix lumen as well as adult worms and eggs, into the abdominal cavity. The absence of an inflamatory exudate of the suppurative type generally found in acute appendicitis was observed; the discrete ínflammatory reaction and the dissociation of the tissues ledt o the idea of a lytic ferment produced by the worms. It is possible that the presence of an infectious hepatites, in the present case, diminishing the patient resistence, plays a role in makíng easy the appendix perforation. (AU)
