El método clínico en tiempos de COVID-19 / The clinical method in times of COVID-19

RESUMEN

RESUMEN Fundamento la enfermedad COVID-19 requiere un enfoque integral para la atención médica. El método clínico proporciona las herramientas adecuadas para jerarquizar el proceso de diagnóstico, tratamiento y seguimiento de los pacientes. Objetivo: profundizar en el empleo del método clínico durante la pandemia de COVID-19. Métodos: se realizó una revisión bibliográfica sistemática para desarrollar un análisis crítico reflexivo del contenido de documentos. La búsqueda fue realizada en las bases de datos SciELO, Google académico, Pubmed entre los meses de octubre del 2021 a enero del 2022; las palabras claves utilizadas fueron métodos, medicina clínica, coronavirus, SARS‐CoV‐2, nuevo coronavirus 2019. Tras la identificación de los estudios preseleccionados, se seleccionaron 25 y se tomó de ellos los contenidos de mayor importancia. Resultados: el uso inteligente del método clínico durante tiempos de contingencia y el uso de medios auxiliares tienen una función decisiva en el diagnóstico médico, son capaces de aportar evidencia más allá de la sensibilidad normal del examinador. Diversos factores entorpecen el proceso el resquebrajamiento de la relación médico-paciente, el menosprecio del valor del interrogatorio, el examen físico y la utilización irracional de la tecnología para llegar al diagnóstico. Conclusiones: se constató cómo a través de la experiencia de los equipos de trabajo y el manejo clínico individualizado, además de las habilidades desarrolladas y el conocimiento adquirido con el objetivo de garantizar la accesibilidad rápida y efectiva de la atención integral a cada paciente a través del método clínico, este constituye una herramienta segura para el manejo individual y comunitario de las enfermedades.

ABSTRACT Introduction: COVID-19 disease requires a comprehensive approach to medical care. The clinical method provides the appropriate tools to prioritize the process of diagnosis, treatment and follow-up of patients. Objective: to deepen the use of the clinical method during the COVID-19 pandemic. Methods: A systematic bibliographic review was carried out to develop a reflexive critical analysis of the content of documents. The search was carried out in the SciELO, academic Google, and Pubmed databases from October 2021 to January 2022; the keywords used were methods, clinical medicine, coronavirus, SARS‐CoV‐2, new coronavirus 2019. After identifying the preselected studies, 25 were selected and the most important contents were taken. Results: the intelligent use of the clinical method during times of contingency and the use of auxiliary aids have a decisive role in medical diagnosis, they are able to provide evidence beyond the normal sensitivity of the examiner. Various factors hinder the process the breakdown of the doctor-patient relationship, the underestimation of the value of the questioning, the physical examination and the irrational use of technology to reach the diagnosis. Conclusions: it was found that the clinical method constitutes a safe tool for the individual and community management of diseases, applying the experience of work teams and individualized clinical management, the skills developed and the knowledge acquired with the aim of guaranteeing accessibility rapid and effective comprehensive care for each patient.
