Os cuidados institucionalizantes a pessoas idosas no Rio Grande do Norte / Institutional care for elderly people in Rio Grande do Norte

RESUMO

As Instituições de Longa Permanência para Idosos (ILPI) constituem uma modalidade assistencial de caráter domiciliar, coletivo e com cuidados longitudinais, destinada às pessoas com idade igual ou superior a 60 anos, com ou sem suporte familiar, onde elas devem viver em condição de liberdade, dignidade e cidadania. Entretanto, na história da institucionalização da velhice, as ILPI foram também conhecidas como asilos que se configuraram, por vezes, como espaços de exercício da caridade cristã e de amparo da pobreza individual e familiar, com regras para limitar e homogeneizar as atividades dos idosos abrigados, privando-os do convívio social e familiar, o que ainda hoje se observa em algumas ILPI. Nesse sentido, surgem indagações sobre os cuidados oferecidos, cuja reflexão incide no processo de manicomialização das pessoas idosas institucionalizadas. Objetivou-se analisar os cuidados oferecidos às pessoas institucionalizadas. Esta pesquisa possui abordagem mista, caráter descritivo e exploratório e foi realizada nas ILPI filantrópicas do Estado do Rio Grande do Norte, cadastradas no Sistema Único da Assistência Social (SUAS) e reconhecidas pela Vigilância Sanitária. O trabalho de campo foi realizado no primeiro semestre de 2019, através de roteiro de observação direta não participante, aplicação de questionários estruturados e entrevistas semiestruturadas com atores envolvidos no processo de trabalho das ILPI. A análise dos dados levantou aspectos que atravessam e condicionam os cuidados oferecidos aos residentes. Assim, as discussões estão organizadas em três capítulos, a saber: 1- Perfil das Instituições de Longa Permanência para Idosos e qualidade de vida dos residentes; 2- A institucionalização das pessoas idosas: entre cuidados curativos e cuidados alienantes; e, 3- Viver ou conviver? Desafios e possibilidades de cuidados às pessoas idosas nas Instituições de Longa Permanência. Portanto, a reflexão sobre os cuidados oferecidos nas ILPI evidencia uma descontinuidade no diálogo entre as redes de assistência social e de saúde, produzindo a necessidade de transformação no acolhimento de longa permanência dos idosos. As redes precisam se unir para um planejamento efetivo referente às políticas públicas de proteção aos idosos, evidenciando estratégias que supram as necessidades da sociedade brasileira em envelhecimento, por meio de serviços que ofereçam suporte aos cuidados longitudinais e promovam a desinstitucionalização de pessoas idosas das ILPI que tenham a possibilidade de voltarem a conviver no seu seio familiar e comunitário (AU).

ABSTRACT

The Homes for the Aged (ILPI) are a type of home assistance, collective and with longitudinal care, aimed at people aged 60 years or over, with or without family support, where they must live in conditions of freedom, dignity and citizenship. However, in the history of the institutionalization of old age, ILPI were also known as asylums that were configured, at times, as spaces for the exercise of Christian charity and the protection of individual and family poverty, with rules to limit and homogenize the activities of sheltered elderly people, depriving them of social and family life, which is still observed in some ILPI. In this sense, questions arise about the care offered, whose reflection focuses on the process of asylum for institutionalized elderly people. The objective was to analyze the care offered to institutionalized people. This research has a mixed approach, descriptive and exploratory and was carried out in the philanthropic ILPI of the State of Rio Grande do Norte, registered in the Unified Social Assistance System (SUAS) and recognized by the Health Surveillance. The fieldwork was carried out in the first semester of 2019, through a script of direct nonparticipant observation, application of structured questionnaires and semi-structured interviews with actors involved in the ILPI work process. The analysis of the data raised aspects that cross and condition the care offered to residents. Thus, the discussions are organized into three chapters, namely: 1- Profile of Long Term Care Institutions for the Elderly and residents' quality of life; 2- The institutionalization of the elderly: between curative and alienating care; and, 3- Live and coexist? Challenges and possibilities of care for the elderly in Long Term Care Institutions. Therefore, the reflection on the care offered at the ILPI shows a discontinuity in the dialogue between the social and health care networks, producing the need for transformation in the long-term care of the elderly. The networks need to come together for effective planning regarding public policies for the protection of the elderly, showing strategies that meet the needs of Brazilian society in aging, through services that support longitudinal care and promote the deinstitutionalization of elderly people in the ILPI who have the possibility to return to live together in their family and community (AU).
