La gran multiparidad (GM) está fuertemente relacionada con causas de incapacidad, enfermedad y muerte entre mujeres en edad reproductiva en países en desarrollo, así como con alteraciones en la salud feto-neonatal de gran relevancia social y familiar. Se realizó un estudio cualicuantitativo descriptivo, inferencial cuyo objetivo general fue analizar las diferencias sociodemográficas entre mujeres embarazadas GM de aquellas que no lo son, durante 2008-2009, y explorar dimensiones socioculturales relacionadas con la condición de GM en la comunidad hospitalaria del HMI Ramón Sardá. Para ello se utilizaron variables obtenidas de fuentes de datos primarias (entrevista semiestructurada elaborada ad-hoc) y secundarias (historia clínica perinatal e internación). Resultaron variables asociadas a GM edad al primer parto, connotación negativa hacia métodos anticonceptivos y nacionalidad. Otras asociaciones significativas fueron antecedentes de abuso sexual y maltrato, bajo nivel educativo y migraciones, así como pobreza estructural. No tuvo peso el antecedente de familia numerosa o edad de la madre de la GM. Se propone considerar estos aspectos al momento de elaborar estrategias de intervención y Programas de apoyo de estas mujeres particularmente vulnerables; así como capacitación del equipo de salud para reducir sus riesgos. A nivel comunitario, posibilitar la transferencia de los resultados y proyectar la experiencia a instituciones hospitalarias de similares características.
ABSTRACT
The great parity (GM) turns out to be a risk factor strongly associated with causes of disability, illness and death among women of reproductive age in developing countries as well as important alterations in fetal-neonatal health. We conducted a descriptive and inferential qualitativequantitative study, whose main objective is to analyze the demographic differences among GM pregnant women from those who are not, during the period 2008-2009. And to explore socio-cultural dimensions related with GM condition into the Hospital Materno Infantil Ramon Sarda’s patients. We have obtained variables from primary data sources (ad-hoc prepared semi-structured interview) and secondary data sources (Perinatal history). The most associated with GM variables were age at the first birth, negative connotation to contraception and nationality. Other significant associations were history of sexual abuse and abuse, low educational attainment, migration, and structural poverty. Large family and GM mother age at the first delivery were not associated. It is proposed to consider these aspects in the following areas • To complement a boarding from socio-demographic and cultural aspects, to develop intervention’s strategies as a Program of integral attention of this particularly vulnerable group of women. • To train the health staff in order to reduce their risks • To transfer the results and to project the experience to hospitals with similar characteristics.
