RESUMEN
La sífilis es una enfermedad crónica e infecciosa tan antigua como la humanidad. Su etapa secundaria se caracteriza fundamentalmente por lesiones en piel y mucosas, pero en algunas ocasiones puede afectar órganos importantes como los ojos y el hígado, donde puede provocar una hepatitis sifilítica. Se presenta el caso de un paciente masculino con manifestaciones cutáneas y en las mucosas de ictericia, cuyo comienzo fue concomitante con lesiones en piel sugestivas de ser secundarias a sífilis. Las principales manifestaciones en piel eran pápulas eritematosas y coloración amarilla. Se constató serología para enfermedades venéreas positiva a 256 diluciones lo que corroboró el diagnóstico de sífilis en su fase secundaria. El paciente fue tratado por esta causa y los síntomas clínicos no remitieron, por lo que se le realizó una colangiopancreatografía retrógrada endoscópica que informó presencia de proceso tumoral de páncreas, que invadía este órgano en su totalidad e infiltraba vías biliares. Se publica el caso por haberse presentado sospecha de una posible hepatitis sifilítica, condición poco frecuente en nuestro medio, junto a un tumor de páncreas y para destacar la importancia del diagnóstico diferencial de la hepatitis sifilítica.
ABSTRACT
Syphilis is a chronic and infectious disease as old as mankind. Its secondary stage is characterized mainly by skin lesions and mucous , but in some cases it can affect important organs as the eyes and the liver, where it can cause syphilitic hepatitis. A case of a male patient with cutaneous and mucous manifestations of jaundice is presented, whose onset was concomitant with skin lesions suggestive of a secondary syphilitic lesion. The patient was treated for this cause and the clinical symptoms did not improve, so he was performed an endoscopic retrograde cholangiopancreatography which reported the presence of a pancreatic tumor process, which invaded entirety this organ and infiltrated bile ducts. The case is presented as suspicion of possible syphilitic hepatitis, a rare condition in our country, with a pancreas tumor and to highlight the importance of the differential diagnosis of syphilitic hepatitis.
