Análise do papel da Secretaria Municipal de Saúde do Rio de Janeiro na implantação do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica - PMAQ-AB / Analysis of the role of the Municipal Health Secretariat of Rio de Janeiro in the implementation of the National Program for Improving Access and Quality of Primary Care - PMAQ-AB

RESUMO

Esta pesquisa tem como objetivo central a análise do papel da Secretaria Municipal de Saúde (SMS) na implantação do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) no município do Rio de Janeiro. Para tal foi realizado um estudo de caso, exploratório e qualitativo por meio de pesquisa documental e entrevistas com atores-chave da Secretaria Municipal. Foi possível a identificação do contexto em que se deu a implantação do programa no âmbito municipal; das ações para a implantação do PMAQ-AB no Rio de Janeiro; dos fatores que dificultaram e que favoreceram a implantação deste; e uma análise preliminar sobre o jogo social da SMS, em um contexto de transformação A SMS necessitou realizar reformas administrativa, organizacional e assistencial para impulsionar a ESF, em processo tardio e concomitante à expansão da mesma, que coincidiu com a implantação no PMAQ-AB no município. Algumas ações identificadas para a implantação do PMAQ-AB foram a valorização da atenção primária, o apoio da SMS e a clara decisão política nesse processo. Fatores favoráveis à implantação do programa foram a sensibilização realizada pela SMS junto aos profissionais envolvidos com o programa através das Coordenadorias de Atenção Primária à Saúde (CAP) e o apoio institucional e matricial proporcionado às equipes de saúde. Já os fatores que dificultaram a implantação do programa foram o atraso na devolução do resultado das certificações pelo Ministério da Saúde e dificuldade de se trabalhar um programa avaliativo. A atuação da secretaria na implantação do programa, no jogo social e as consequências disso foram analisadas com base na Teoria do Jogo Social, utilizando o triângulo de ferro de Matus. No jogo social os atores agem conforme diferentes intenções, no qual o uso de mecanismos de planejamento e gestão foram limitados. ABSTRACT

This research aims to analyze the role of the Municipal Health Secretary in the implementation of the National Program for Improving Access and Quality of Primary Health Care - PMAQ-AB in the Rio de Janeiro’s city. This is a case study with approach qualitative and exploratory through documents research and interviews with key actors of the municipal health secretary. The actions were identified for the implementation of the program, nonfavored factors and favored factors of the PMAQ-AB deployment in Rio de Janeiro and the preliminary analysis of the social game, in a context of transformation. The SMS needed to perform administrative, organizational and health model reforms to improve the ESF, concurrently with the expansion, there was the deployment in PMAQ-AB in the city. Some actions identified for the PMAQ-AB implementation were valuing primary health care, the support of the Municipal Secretary and the clear political will in this process. Factors favorable to the implementation of the program were the sensitization carried out by SMS to program’s professionals, also through the Coordination of Primary Health Care (CAP) and institutional support and matrix provided to health teams. As for the factors that interfere with implementation of the program were the delay in returning the result of certification by the Ministry of Health and difficulty of working an evaluation program. The role of the health department to implement the program, the social game and the consequences were analyzed based on the Theory of Social Game, using Matus iron triangle. In the social game actors act as different intentions, the use of planning and management mechanisms were limited.
