Desafios da integralidade na assistência: o itinerário terapêutico de mães com sofrimento psíquico grave / Challenges in health care integrality: the therapeutic journey of mothers with intense psychic suffering

RESUMO

Este estudo teve como objetivo mapear o Itinerário Terapêutico (IT) de mulheres/mães que apresentam uma condição de sofrimento psíquico grave, observando a integralidade no cuidado, respeitando sua condição de mulher e sua fragilidade psíquica. Valeuse do método da história de vida com a condução de entrevistas semiestruturadas, gravadas em áudio e vídeo, e posteriormente transcritas. Foram realizadas entrevistas com seis mulheres e, a partir dos dados produzidos, foram defnidas quatro categorias temáticas 1) Equipamentos e Estratégias Formais de Tratamentos; 2) Equipamentos e Estratégias Informais de Tratamento; 3) Rede de Suporte Social e 4) Engajamento Político-Social. Nas análises empreendidas, percebe-se que os IT's são construídos de modos diversos e fragmentados. Em linhas gerais, as mulheres, com o apoio de alguns familiares, amigos e profssionais são protagonistas na construção do itinerário terapêutico percorrido, no entanto, a integralidade que, neste caso, pressupõe uma oferta de cuidados que ultrapassa os esforços individuais e demanda ações articuladas entre a saúde mental e a saúde materno-infantil mostrou-se frágil, sendo observadas situações de desamparo e ausência de uma oferta de um cuidado mais articulado para a mulher, considerando sua condição de mãe e de alguém cuja trajetória de vida está marcada pelo sofrimento psíquico grave.

ABSTRACT

This paper aims at mapping therapeutic itinerary (IT) of women/mothers with severe psychic suffering, observing the integrality in care in relation to the account for their female condition and psychic fragility. We used the life history method with semi-structured interviews, recorded in audio and video and later transcribed. We have performed interviews with six women and have created four theme categories 1) Equipments and formal strategies of treatment; 2) Equipments and informal strategies of treatment; 3) Social support network and 4) Political and social engagement. Throughout analyzes it was found that the it was constructed in various and fragmented modes. In general, women supported by family, friends and professional were protagonists in constructing therapeutic itinerary. However, the integrality (that is based on an offer of care beyond individual efforts and that demands articulated actions between mental health and the maternal/child health) proved to be fragile. It was also observed lack of an articulated care and absence of a comprehensive approach related to their condition as mothers and severe psychic suffering subjects.
