RESUMO
Espondilodiscite é a doença infecciosa da coluna vertebral, que acomete o disco e os corpos vertebrais contíguos. É uma patologia que se manifesta com dores difusas, vagas, de caráter insidioso, geralmente sem febre ou outros indícios de infecção, cujo diagnóstico permanece difícil e isto implica em retardo diagnóstico e elevado potencial de morbidade. A etiopatogenia principal é a via hematogênica, mas também ocorre por contiguidade e inoculação direta devido a traumas e procedimentos cirúrgicos e diagnósticos. A investigação complementar é inespecífica e entre os exames de imagem, a RNM é o mais sensível para detecção precoce. O diagnóstico definitivo é através da identificação e cultura do material coletado por punção biópsia. O diagnóstico precoce acurado e a instituição precoce da terapêutica são fatores decisivos para o desfecho favorável e para afastar a necessidade de intervenção cirúrgica e a ocorrência de sequelas.
ABSTRACT
Spondylodiscitis is an infectious disease of the spine that affects the disc and the adjacent vertebral bodies. It is a disease that manifests itself in pain diffuse, vague, insidious, usually without fever or other signs of infection, whose diagnosis remains difficult and this leads to delayed diagnosis and high potential for morbidity. The main etiology is hematogenous, but also occurs by contiguity and direct inoculation due to trauma and surgical and diagnostic procedures. Complementary research is nonspecific and among imaging, MRI is more sensitive for early detection. Definitive diagnosis is by culture and identification of the material collected by punch biopsy. Early diagnosis and accurate early institution of therapy are decisive factors for favorable outcome and to avoid the need for surgical intervention and the occurrence of sequelae.
