Efeitos da cinesioterapia sobre o equilíbrio e a marcha de uma paciente com distonia de Segawa / Effects of kinesiotherapy on balance and gait in a patient with Segawa dystonia

RESUMO

Objetivo ­ Avaliar o efeito de um protocolo de exercícios cinesioterapêuticos sobre o equilíbrio e a marcha de uma paciente com DRD. A Distonia de Segawa é conhecida também como Distonia Responsiva à Levodopa (DRD). Trata-se de um tipo de distonia progressiva, com início na infância e adolescência, que provoca alterações na marcha, equilíbrio, postura e coordenação. Métodos ­ Participou do estudo uma paciente, 25 anos, sexo feminino, com diagnóstico de DRD. O equilíbrio e a marcha foram avaliados por meio da Escala de Equilíbrio de Berg, do Teste de Tinetti e da análise da marcha utilizando as medidas do tamanho do passo, da passada e da base de sustentação, antes e após o tratamento. A paciente foi submetida a 15 sessões de cinesioterapia, foram realizados exercícios de fortalecimento muscular com ênfase nos membros inferiores e para potencializar o equilíbrio e a marcha. Resultados ­ Houve melhoras significativas do equilíbrio pela Escala de Equilíbrio de Berg (p=0,03) e da marcha para as medidas do tamanho do passo (p=0,03), da passada (p=0,04) e da base de sustentação (p=0,01), porém não houve melhoras significativas para o Teste de Tinetti. Conclusão ­ O protocolo de tratamento proposto baseado na cinesioterapia proporcionou melhoras do equilíbrio e da marcha para a paciente em estudo.

ABSTRACT

Objective ­ The aim of this study was to evaluate the effect of kinesiotherapy on balance and gait in a patient with DRD. The Segawa Dystonia, also known as Levodopa Responsive Dystonia (DRD) is a type of progressive dystonia beginning in childhood and adolescence that causes changes in gait, balance, posture and coordination. Methods ­ Participated in the study, a patient 25-year-old female with DRD. The balance and gait were evaluated by the Berg Balance Scale, Tinetti Test and the gait analysis using the measurements of the step size, stride and the base of support before and after treatment. The patient underwent 15 sessions of kinesiotherapy; strengthening exercises were performed emphasizing lower limbs and also activities to enhance balance and gait. Results ­ There were significant balance improvements using Berg Balance Scale (p=0.03) and gait to the measurements of the step size (p=0.03), stride (p=0.04) and the base of support (p=0.01), but there was no significant improvement for the Tinetti test. Conclusion ­ It is concluded that after the completion of the treatment protocol the patient showed improvements in balance and gait.
