RESUMEN

Introducción:

Las adenoides integran el anillo de Waldeyer, está situado en la porción postero-superior de la nasofaringe. Este tejido, puede hipertrofiarse por infecciones crónicas y recurrentes, procesos alérgicos. Es una de las causas más frecuentes de obstrucción nasal y de indicación quirúrgica en niños.

Objetivo:

Conocer las comorbilidades que afectan a pacientes de 1 a 14 años con rinoadenoiditis, en relación al grado de hipertrofia de adenoides y la presencia o no de atopia. Materiales y

métodos:

Estudio observacional, descriptivo, de prevalencia, retrospectivo; a partir de historias clínicas de Sección de Alergia e Inmunología del Hospital Misericordia; entre Septiembre de 2010 a 2013 sobre pacientes de 1 a 14 años con diagnóstico de Rinoadenoiditis. Se valoróhipertrofia adenoidea (HA) con índice de Fujioka.Estadística con Prisma 5 GrafPad.Se estudiaron las variables edad, sexo, sensibilidad a áero- alérgenos, VSG, neutrófilos, eosinófilos, IgE, comorbilidades.

Resultados:

Se estudiaron 102 pacientes, 46 femeninos y 56 masculinos. La edad en sexo femenino fue 5.48+ 3 años y en masculino 5.6+2,47 años.56 pacientes presentaron hipertrofia adenoidea (HA) grado I, 33 grado II y 13 grado III. IgE sérica fue de 218.8 + 29.3.VSG, fue engrado I 4.89±2,39 mm; grado II 11.15±9.04 mm; grado III 6.79±2.19, mm p=0.0129.Los eosinófilos fueron en grado I 315±33 x mm3, grado II 123±23,6 por mm3; grado III 114± 25.5 por mm3 (p<0.0001). Los neutrófilos fueron en grado I 4681±174 x mm3; grado II 5231±230x mm3, grado II 4576±309 x mm3, NS.Comorbilidades Grado I RS y As 21, RS 19, As 9, RS As Ot 2, RsOt 2 Faringitis 3. Grado II asma 3, RS Asma 10, RS AS Otitis 2, RS 14, RS/ Ot 3, Faringitis 1. Grado III RsAs 1, Rs 1, As 1, RS AsOt 4, RsOt 6, p=0.0323

Conclusiones:

La hipertrofia adenoidea grado I resultó la más frecuente, con recuento mayor de eosinófilos y mayor sensibilidad a Dermatofagoides y otros alérgenos. La VSG está aumentada significativamente en hipertrofia grado II y III, lo cual indicaría infecciones sobre- agregadas. Las co-morbilidades más frecuentes fueron rinosinusitis en los tres grados; asma en el grado I y otitis en el grado III.
ABSTRACT

Introduction:

Adenoids make up the Waldeyer ring, which is located in the posterosuperior part of the nasopharynx. This tissue can suffer hypertrophy due to chronic and recurring infections (allergic processes). This is one of the most common causes of nasal obstruction and of surgical indication in children. Goal To recognize the comorbidities that affect patients from 1 to 14 years old who suffer rhinoadenoiditis related to the grade of adenoids' hypertrophy and the presence or absence of atopy. Methods and materials This study is observational, descriptive and retrospective and it was taken from clinical histories from the Allergy and Immunology Department of Hospital Misericordia, from September 2.010 to 2.013, on patients from 1 to 14 yeras old suffering from rhinoadenoiditis. Adenoid hypertrophy (AH) was valued with an index of Fujioka (statistics with prism 5 grafpad). Some variables were studied age, sex, aero-alllergen sensibility, ESR, neutrophyls, eosinophyls, IgE, comorbidities.

Results:

102 patients were studied, 46 female and 56 male. Age in female was 5,48 + 3 years old and in male it was 5,62 + 2.47 years old. 56 patients showed adenoid hypertrophy (AH) grade I, 33 grade II and 13 grade III. IgE was of 218,80 + 29,3 ESR at grade I 4,89 + 2.39mm grade II 11,15 _+ 9,04mm; grade III 6,79_+ 2,19mm p=0,0129. The eosinophyls were at grade I 315_+ 33 per mm3; grade II 123_+23.6 per mm3; grade III 114_+ 25,5 per mm3 (p0,0001). Neutrophyles were at grade I 4681_+174 per mm3; grade II 5231_+230 per mm3, grade III4576 _+ 309 per mm3, NS. Comorbidities grade I RS and As 21, RS 19, As 9, RS As Ot 2, RsOt 2 pharyngitis 3. Grade II asthma 3, RS Asthma 10; RS AS Otitis 2, RS 14, RS/Ot 3, pharyngitis 1. Grade III RsAs 1, Rs 1, As 1, RS AsOt 4, RsOt 6, p=0,0323.

Conclusions:

The adenoid hypertrophy grade I turned out to be the most common one with more eosinophyles and more sensibility to dermatofagoids and other allergens. The ESR is increased significantly in hypertrophy grade II and III, which would show overadded infections. The most common comorbidities were rhinosinusitis at all grades; asthma at grade I and otitis at grade III.
