Adherencia terapéutica y factores influyentes en pacientes con diabetes mellitus tipo 2 / Therapeutic adherence and influencing factors in patients with type 2 diabetes mellitus

RESUMEN

Introducción: la baja adherencia al tratamiento constituye un problema para el control de la diabetes mellitus. Es una enfermedad presente por su impacto en el cuadro de morbilidad y mortalidad del país y la provincia de Pinar del Río. Objetivo: identificar los niveles de la adherencia terapéutica y los factores influyentes en la adherencia deficitaria en una muestra de pacientes diabéticos. Métodos: se realizó un estudio descriptivo de corte transversal con diseño mixto. Se trabajó con una muestra de 143 pacientes atendidos en el Grupo Básico de Trabajo 1 del Policlínico Universitario Pedro Borrás Astorga de la ciudad de Pinar del Río en el período de junio-septiembre del 2013. Se utilizó el cuestionario MBG para evaluar la adherencia terapéutica, una encuesta sobre conocimientos acerca de la enfermedad y el tratamiento y una entrevista estructurada. Resultados: predominó el nivel de adherencia parcial en el 74,1 por ciento de los pacientes. La mayor contribución a la adherencia terapéutica deficitaria la aportó el componente implicación personal. Los factores personales relativos a los conocimientos sobre la dieta y la motivación hacia el tratamiento fueron las principales variables con influencia sobre la adherencia terapéutica deficitaria. Conclusiones: la adherencia terapéutica de las personas con diabetes mellitus tipo 2 se presentó en sus tres niveles, con predominio del nivel parcial de adherencia. Los factores que condicionan la adherencia deficitaria están relacionados con la falta de conocimientos sobre la dieta a seguir y la ausencia de motivación por realizar adecuadamente el tratamiento(AU)

ABSTRACT

Introduction: Low adherence to treatment is a problem for the management of diabetes mellitus. The disease is present for its impact on the morbidity and mortality of the country and the province of Pinar del Río. Objective: To identify the levels of therapeutic adherence and the influencing factors in insufficient adherence in a sample of diabetic patients. Methods: We carried out a descriptive, cross-sectional study with a mixed design. We worked with a sample of 143 patients treated in the basic working group number 1 of Pedro Borrás Astorga University Polyclinic of the city of Pinar del Río in the period from June to September 2013. We used the MBG questionnaire to assess therapeutic adherence and a survey about knowledge about the disease and treatment and a structured interview. Results: Partial adherence prevailed in 74.1 percent of patients. The greatest contribution to the lack of therapeutic adherence was provided by the component of personal involvement. Personal factors related to knowledge about diet and motivation towards treatment were the main variables with influence on insufficient adherence to treatment. Conclusions: The therapeutic adherence of people with type 2 diabetes mellitus was presented in its three levels, with predominance of the partial level of adherence. The factors that condition insufficient adherence are related to the lack of knowledge about the diet to follow and the lack of motivation to adequately perform the treatment(AU)
