Capacitação multiprofissional em cuidados paliativos precoces / Multiprofessional training in early palliative care

RESUMO

Apesar de todos os esforços empregados na detecção precoce, o câncer de mama apresenta altas taxas de mortalidade atribuídas ao diagnóstico tardio. Conta-se, no cenário oncológico atual, com o surgimento de novas drogas e tecnologias para prolongamento de vida que trouxeram dilemas éticos, como o momento ideal para encaminhamento de pacientes aos cuidados paliativos (CP). Como reflexo, tem-se uma grande demanda de pacientes sem possibilidades de cura que apresentam características e demandas específicas e a dificuldade de início precoce dos CP. Este estudo tem como objetivo geral desenvolver capacitação multiprofissional em CP precoces. Os objetivos específicos são identificar, descrever e discutir a concepção de CP e qualidade de vida (QV) na percepção da equipe multiprofissional e sua articulação prática. Trata-se de um estudo descritivo, de abordagem qualitativa, realizado após aprovação do Comitê de Ética em Pesquisa do Instituto Nacional de Câncer, sob Certificado de Apreciação Ética nº 17685913.0.0000.5274. A coleta de dados ocorreu por entrevistas semiestruturadas e a análise com auxílio do software ALCESTE versão 4.9. Foram encontradas três categorias Concepções de QV e CP; A teia das relações profissional/paciente e trabalho em equipe; A prática dos CP. Os CP foram associados a aspectos relacionados à doença e ao paciente. A QV esteve associada à singularidade, saúde e manutenção da funcionalidade. A relação profissional/paciente está comprometida e o trabalho em equipe foi visto como importante. O produto gerado foi a elaboração do Programa de Capacitação Multiprofissional em Cuidados Paliativos Precoces. Ao final, questões complexas emergiram da pesquisa de campo, que foram suficientes para identificar lacunas no conhecimento e amparar a elaboração da capacitação voltada para o público alvo

ABSTRACT

Despite all the efforts made in the early detection, breast cancer has high mortality rates attributed to late diagnosis. In the current oncological scenario, with the emergence of new drugs and technologies to prolongation of life, emerged ethical dilemmas, as the ideal time for referral of patients to palliative care (PC). As a reflection, there is a great demand of patients without healing possibilities of cure that have characteristics and specific demands and difficulty of the early onset PC. This study has the general objective to develop multiprofessional training in early PC. The specific objectives are to identify, describe and discuss the design of PC and quality of life (QOL) in the perception of the multidisciplinary team and the practice articulation. This is a descriptive study of qualitative approach, carried out after approval by the Research Ethics Committee of the National Cancer Institute, under Certificate of Appreciation Ethics No. 17685913.0.0000.5274. The data were collected by semi-structured interviews and analyzed using the Alceste software version 4.9. Three categories were found Conceptions of QOL and PC; The web of relations professional/patient and teamwork; The practice of PC. The PC were associated with aspects of the disease and the patient. QOL was associated to uniqueness, health and maintenance functionality. The professional/patient relationship is committed and teamwork was seen as important. The product generated was the preparation of the Multidisciplinary Training Program in Palliative Care Early. In the end, complex issues emerged from the field of research, which were sufficient to identify gaps in knowledge and support the development of focused training for the target audience
