Avaliação do conhecimento médico sobre terapia nutricional parenteral em um hospital público do Distrito Federal / Evaluation of medical knowledge about parenteral nutrition therapy in a public hospital in the Federal District

RESUMO

Introdução: A Terapia Nutricional Parenteral (TNP) visa à otimização do estado nutricional com diminuição das complicações, sendo indicada em diversos casos. A falta de conhecimento médico sobre o tema é um fator contribuinte na precariedade do serviço e é, muitas vezes, essencial em um tratamento clínico mais efetivo. Existe reconhecimento entre os médicos sobre a importância da intervenção nutricional, porém poucos procuram atualizações sobre o tema. O objetivo deste trabalho é avaliar o conhecimento médico sobre TNP em um hospital público do Distrito Federal. Método: Foram aplicados questionários contendo 20 questões abordando os itens benefício, indicação, atribuição médica, prescrição, acompanhamento, riscos e as atribuições dos demais profissionais de saúde na TNP a 20 médicos atuantes em 5 clínicas distintas de um hospital público do Distrito Federal. Foi realizada a análise descritiva para caracterizar a frequência das respostas. Resultados: Dos participantes, 50% não se sentiam preparados para trabalhar com Nutrição Parenteral (NP), sendo a maior porcentagem encontrada na clínica médica. Houve menor percentual de acerto nos itens indicação e prescrição da TNP, sendo indicado como foco em um possível treinamento pela Equipe Multiprofissional de Terapia Nutricional (EMTN). Com relação às clínicas, as que requerem maior atenção quanto aos cuidados com a NP é a Unidade de Terapia Intensiva Neonatal (UTIN) e a clínica médica, respectivamente. Conclusão: A equipe médica necessita de treinamento, visto que metade dos participantes não se sente preparada para trabalhar com a TNP. Os principais tópicos a serem abordados são as suas indicações e prescrição.

ABSTRACT

Introduction: A Parenteral Nutrition Therapy (PNT) aims to optimize nutritional status with reduced complications and is indicated in several cases. The lack of medical knowledge on the subject is a contributing factor in the precariousness of the service and is often essential in a more effective clinical treatment. There is recognition among doctors about the importance of nutritional intervention, but a few seek updates on the subject. The objective of this work is to rate the medical knowledge about PNT In a public hospital of the Federal District. Methods: Questionnaires containing 20 questions addressing the items benefit, indication, medical attribution, prescription, monitoring, risks and attributions of others health professionals in the PNT, were applied in 20 physicians working in 5 different clinics of a public hospital at federal district. A descriptive analysis was performed to characterize a frequency of responses. Results: Of the participants, 50% did not feel prepared to work with Parenteral Nutrition (PN), being greater percentage found in the medical clinic. There was a lower percentage of accuracy in the items indication and prescription of PNT, being indicated as a focus on possible Multiprofessional Nutrition Therapy Team (MNTT) training. With regard to clinics, such as that require greater attention regarding health care with a PN is a Neonatal Intensive Care Unit (NICU) and a medical clinic, respectively. Conclusion: The medical team needs training since half of the participants do not feel prepared to work with the PNT. The main topics to be addressed are their indications and prescription. (AU)
