RESUMEN
Introducción:
Las lesiones Parciales Articulares del Manguito Rotador ( PASTA) cuando son de mas del 50% del espesor del tendón son de tratamiento quirúrgico. Se pueden reparar mediante técnicas trans-tendón o completando la lesión para luego repararla. Ambas técnicas presentan resultados favorables. El objetivo de la presente revisión sistemática con meta-análisis es comparar los resultados clínicos y el riesgo de re-ruptura. Materiales yMétodos:
Se realizo una búsqueda sistemática en las bases de datos de Pubmed, EMBASE, CINAHL, LILACS de todo las publicaciones referentes a las lesiones PASTA utilizando. Criterios de inclusión debían ser estudios clínicos de nivel de evidencia I o II, con resultados funcionales uando escalas validadas, un seguimiento mínimo de 12 meses.Resultados:
Se incluyeron cuatro trabajos con 274 pacientes. En la escala de Constant no se observó diferencia entre los grupos (p=0.80), La escala de ASES no se observó una diferencia estadísticamente significativa de (p=0.91) pero el riesgo de re-ruptura fue menor en las reparaciones transtendón (p=0.05).Conclusión:
El presente meta-análisis sugiere que la reparación transtendinosa es mejor que completar la lesión para luego repararla en las lesiones parciales de manguito rotador que toman mas del 50% del espesor del tendón, tomando como punto de corte el riesgo de re-ruptura.ABSTRACT
Introduction:
Partial Articular Rotator Cuff tears of more than 50% of the thickness of the tendon are treated surgically. The options are a transtendon repair or to complete the tear and then repair it, both show good results. The objective of the present systematic review with metanalysis is to compare thier clinical results, and complications with focus on the retear risk. Materials andmethods:
Systematic search of PubMed, EMBASE, CINAHL, LILACS was performed searching publications about PASTA Lesion and their repair. Inclusion criteria, Level I or II Clinical Studies with a minimum follow up of 12 months, using validated functional scores.Results:
Four studies with 274 cases were included. No signi cant difference was observed in de functional scores, Constant (p=0,8) or ASES (p=0,91) between the two groups. But there was a signi cant difference between the re tear rate (p<0,05) favoring transtendon repair.Conclusion:
The present systematic review suggest that a transtendon repair is a better option for a PASTA lesion with fewer re tear rate.
