Resumo

Introdução:

A alimentação enteral dos recém-nascidos pré-termo (RNPT) permanece um desafio, pois não se sabe ao certo o que é necessário para se ter uma nutrição que atenda a todas as suas necessidades de crescimento e desenvolvimento. O leite humano supre as necessidades dos RNPT nas primeiras semanas de vida com relação aos fatores de proteção e crescimento com conteúdo proteico; enzimático; energético e imunológico. Entretanto, após esse período, a oferta de nutrientes diminui, principalmente em relação à quantidade de fósforo, proteína e cálcio, sendo por vezes necessária a suplementação do leite com o acréscimo de nutrientes industrializados. Também há dúvidas sobre o seu componente bacteriológico na gênese de processos infecciosos neonatais.

Objetivo:

Verificar a composição de eletrólitos e minerais e realizar a avaliação microbiológica do leite de lactantes a termo antes e após a pasteurização e de leite cru de mães de recém-nascido pré-termo coletado à beira do leito.

Métodos:

Estudo descritivo de corte transversal. Foram dosadas concentrações de cálcio (Ca), fósforo (P), magnésio (Mg), sódio (Na) e potássio (K) em amostras aleatórias de leite humano nos três grupos. As amostras foram analisadas por mineralização ácida assistida por radiação micro-ondas e posteriormente por espectrometria de emissão óptica com plasma indutivamente acoplado e expressas em mg/L, com cálculo de médias e desvio padrão. Para a avaliação bacteriológica foi utilizada a técnica do BGBL, utilizada pela Rede de Banco de Leite. A comparação entre os grupos foi feita por análise de variância (ANOVA)/teste de Tukey. Nível de significância aceito de 5%. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da Unicamp sob o número CAAE 35545314.4.0000.5404 e aprovado pela REBEC RBR- 7994vz e foi assinado o consentimento livre e esclarecido informado dos participantes.

Resultados:

Observou-se uma redução significante dos teores de Ca (259,4±96,8 x 217,0±54,9; p=0,003), P (139,1±51,7 x 116,8±33,3; p=0,004) e K (580,8±177,1,4 x 470,9±109,4; p<0,0001) após a pasteurização. As amostras de leite cru colhidas à beira do leito apresentaram teores estatisticamente mais elevados de Na ¿ 2 vezes - que o leite a termo de doadora. Ca e P só atingem a ingestão recomendada pela European Society of Pediatric Gastroenterology, Hepatology and Nutrition se o leite materno for ofertado em volume maior do que 60 mL a cada 3 horas. Concentrações de magnésio não diferiram entre os grupos. Na avaliação do BGBL pode-se observar que o processo de pasteurização é efetivo e que aproximadamente 50% das amostras colhidas em beira de leito tiveram resultado positivo na avaliação do BGBL

Conclusões:

houve uma redução significativa de Ca, P e K nas amostras após pasteurização, e os valores de Na no leite cru coletado à beira do leito foram superiores àqueles pré-pasteurização. Quanto à análise microbiológica, a pasteurização se mostrou eficiente e a coleta na beira do leito apresentou elevada positividade no BLBG(AU)

Abstract

Introduction:

Enteral feeding of preterm neonates (PN) remains a challenge, because no one knows for sure what is required to have a nutrition that supplies all their needs for growth and development. Human milk supplies the needs of PN in the first weeks of their lives, regarding protection and growth factors such as immunological, energetic, enzymatic and protein contents. However, after this period, the nutrient supplies decrease, mainly in relation to the amount of phosphorus, calcium and protein, being sometimes necessary supplementation of milk with the addition of nutrients. There are also doubts about the bacterial component on its genesis of neonatal infectious processes. Goal To verify the composition of electrolytes and minerals and to carry out the microbiological evaluation of the milk of full term infants before and after pasteurization and raw milk of mothers of preterm babies collecteda at the bedside.

Methods:

descriptive cross-sectional study. Concentrations of calcium (Ca), phosphorus (P), magnesium (Mg), sodium (Na) and potassium (K) have been dosed in random samples of human milk in three groups. The samples were analyzed by acid mineralization assisted by microwave radiation and later by optical emission spectrometry with inductively coupled plasma and expressed in mg/L, with average calculations bases and pattern deviation. For the bacteriological evaluation, the BRILA technique (Brilliant-green Bile Lactose) was used ¿ also used by the Human Milk Bank Network. The comparison between the groups was made by analysis of variance (ANOVA)/ Tukey test. Accepted significance level by 5%. The project was approved by the UNICAMP Research Ethics Committee under the registration number CAAE 35545314.4.0000.5404 and approved by REBEC RBR-7994vz and signed freely and willingly by the informed and consensual participants.

Results:

we observed a significant reduction of the levels of Ca (259.4 ± 96.8 x 54.9 ± 217.0; p = 0.003), P (139.1 51.7 ± x 116.8 ± 33.3; p = 0.004) and K (580.8 ± 177, 4 x 1, 109.4; p < 470.9 ± 0.0001) after pasteurization. Raw milk samples collected at the bedside presented statistically higher levels of On- twice as much of the term donor¿s milk. CA and P only reach the intake recommended by European Society of Pediatric Gastroenterology, Hepatology and Nutrition if breast milk is offered in more than 60 mL every 3 hours. Magnesium concentrations did not differ between the groups. Through the BRILA assessment it can be observed that the process of pasteurization is effective and that approximately 50% of the samples taken at the bedside had a positive result in this evaluation system.

Conclusions:

there was a significant reduction of Ca, P and K in the samples after pasteurization and the values of NA in raw milk collected at the bedside were superior to those of pre-pasteurization. As for microbiological analysis, the pasteurization proved efficient and collection at the bedside presented high positivity in the BRILA system(AU)
