Conhecimento, atitudes e práticas em leishmaniose visceral: reflexões para uma atuação sustentável em município endêmico / Knowledge, attitudes, and practices in visceral leishmaniasis: reflections for a sustainable performance in an endemic municipality

RESUMO

A participação da população é fundamental para efetividade das medidas de prevenção e controle da leishmaniose visceral (LV), sobretudo aquelas relacionadas ao manejo ambiental. A partir do levantamento de conhecimentos, atitudes e práticas (CAP) de moradores residentes em município endêmico para LV, o estudo buscou identificar possíveis caminhos que contribuam para efetividade e sustentabilidade dessas medidas. Entre outubro de 2010 e dezembro de 2013, agentes comunitários de saúde visitaram domicílios localizados em regiões com e sem notificação de casos humanos de LV, observaram as condições ambientais e aplicaram um questionário semiestruturado, que abordava fontes de informação sobre a doença, aspectos epidemiológicos, além de atitude adotada. As respostas foram classificadas em "adequado" ou "inadequado," considerando a orientação do Ministério da Saúde. Nas duas regiões, as principais fontes de informação sobre a LV citadas foram rádio e TV, apenas 6% dos entrevistados relataram os agentes de saúde. Foram identificadas fragilidades no conhecimento, especialmente, sobre sinais e sintomas da LV; uso de adubos orgânicos nas residências e presença de animais dentro dos domicílios. Os resultados apontaram que um possível caminho para superação das lacunas encontradas envolve a interação entre agentes de saúde e comunidade norteada por uma proposta pedagógica contextualizada e dialógica.

ABSTRACT

The participation of the population is fundamental for the effectiveness of prevention and control measures for visceral leishmaniasis (VL), especially those related to environmental management. From the survey of knowledge, attitudes, and practices (KAP) of residents living in an endemic municipality for VL, the study attempted to identify possible ways to contribute to the effectiveness and sustainability of these measures. From October 2010 to December 2013, community health workers visited households located in regions with and without notifications of human cases of VL, observed the environmental conditions, and applied a semi-structured questionnaire that addressed sources of information about the disease, epidemiological aspects, in addition to attitudes adopted. The responses were classified as "appropriate" or "inappropriate" according to the recommendations of the Ministry of Health. In both regions, the main sources of information cited about VL were radio and TV, and only 6% of those interviewed mentioned sources through health workers. Weaknesses have been identified in the knowledge, especially about signs and symptoms of VL, the use of organic compost in homes, and the presence of animals within households. The results showed that a possible way to overcome the gaps involves the interaction between health workers and community guided by a contextualized and dialogic pedagogical proposal.
