El ambiente hospitalario: generador de estigma y rechazo de la gestante con VIH/SIDA / Hospital environment: Generator of stigma and rejection of pregnant women with HIV/aids

RESUMEN

Resumen Introducción. Para las madres del estudio, la gestación transcurre con normalidad hasta que son diagnosticadas con VIH/SIDA. Los estudios sobre vivencias en estos escenarios hospitalarios son escasos. Objetivo. Comprender las experiencias de gestantes que conviven con VIH/SIDA durante su atención en dos instituciones hospitalarias. Materiales y métodos. Estudio cualitativo fenomenológico interpretativo en el que participaron 10 mujeres diagnosticadas con VIH durante la gestación, seleccionadas según pertinencia y adecuación. El tamaño de la muestra fue por saturación teórica. Los datos fueron recolectados a partir de entrevistas a profundidad y analizados con estrategia de Colaizzi. Resultados. La categoría del ambiente hospitalario, como generador de estigma y rechazo a madres con VIH/SIDA, es parte de un proyecto de investigación cuyo tema central fue convivir con VIH/SIDA en la gestación. EL ambiente hospitalario muestra los espacios y actores que rodean a estas mujeres durante el diagnóstico y tratamiento de esta infección. Las participantes se refieren a la primera etapa como "el accidental diagnóstico" y describen la atención como cargada por "la imprudencia de los profesionales". Conclusiones. Las actitudes y comportamientos de profesionales de la salud pueden crear un ambiente hostil para gestantes con VIH/SIDA, agudizando sentimientos negativos, miedos e incertidumbres. No obstante, estas también pueden ofrecerles un escenario amigable y humano que contribuya al cuidado y confianza entre profesional y gestante, ayudando a la madre a enfrentar esta compleja experiencia.

ABSTRACT

Abstract Introduction: For the mothers included in this study, pregnancy is normal until they are diagnosed with HIV/AIDS. Studies on experiences in hospital environments are scarce. Objective: To understand the experiences of pregnant women living with HIV/AIDS during prenatal attention in two hospitals. Materials and methods: Interpretative phenomenological qualitative study, in which ten women, diagnosed with HIV during pregnancy and selected according to relevance and adequacy, participated. The sample size was established by theoretical saturation. Data were collected during in-depth interviews and analyzed through Colaizzi strategy. Results: Categorizing the hospital environment as a generator of stigma and rejection of pregnant women with HIV/AIDS is part of a research project on living with HIV/AIDS during pregnancy. The hospital environment shows the spaces and actors around these mothers during diagnosis and treatment of this infection. Participants refer to the first stage as "accidental diagnosis" and described attention as full of "reckless professionals". Conclusions: Attitudes and behaviors of health professionals can generate a hostile environment for pregnant women with HIV/AIDS, heightening negative feelings, fears and uncertainties. However, they can also offer a friendly and human scenario that contributes to care and trust between professionals and pregnant women, thus helping mothers to cope with this complex experience.
