RESUMO

Objetivo:

Investigar os padrões de produção de melatonina, sono e metabolismo de trabalhadoras gestantes e não gestantes, diurnas e noturnas. Também se constituiu como objetivo do presente estudo, analisar parâmetros de saúde dos recém-nascidos e dos filhos das trabalhadoras estudadas, segundo o turno de trabalho materno.

Métodos:

Foram realizados dois estudos paralelos em um hospital público da cidade de São Paulo. No primeiro estudo foram investigados os padrões de produção de melatonina, sono, antropometria, parâmetros metabólicos, hábitos de vida e alimentares de gestantes trabalhadoras diurnas e noturnas e alguns parâmetros metabólicos dos recém-nascidos. No segundo estudo, foram analisados os padrões de produção de melatonina, sono, antropometria, parâmetros metabólicos, hábitos de vida e alimentares de trabalhadoras diurnas e noturnas e alguns parâmetros metabólicos e alimentares entre os filhos das trabalhadoras, de acordo com o turno de trabalho.

Resultados:

Os produtos deste estudo estão descritos sob a forma de três manuscritos. O primeiro refere-se à hipótese principal do estudo, na qual trabalhadoras sujeitas a alterações dos níveis de melatonina, a qual é reduzida mediante a exposição à luz durante a noite, são mais propensas a desenvolver problemas de saúde, os quais podem ser ocasionados pelo trabalho noturno e comprometem, inclusive, os descendentes das trabalhadoras. O segundo manuscrito apresenta o estudo com as gestantes, e analisa o perfil de produção de melatonina ao longo da gestação, segundo os turnos de trabalho, discutindo-se adicionalmente, os reflexos na saúde dos recémnascidos. O terceiro manuscrito apresenta o estudo realizado com mães e filhos, sob a ótica da influência da exposição à luz à noite durante gestação, e a consequente redução da melatonina com possíveis repercussões no metabolismo dos filhos que já estão na primeira infância ou adolescência.

Conclusão:

A hipótese apresentada no primeiro artigo foi testada nos estudos subsequentes. Em relação ao estudo com gestantes, apresentado no segundo artigo, destaca-se a redução da produção de melatonina nas trabalhadoras noturnas quando comparadas às diurnas, além do escore de Apgar mais baixo entre os recém-nascidos das noturnas. O terceiro artigo, o qual apresenta estudo realizado com as trabalhadoras não gestantes e seus filhos, não verificou diferenças significantes nos parâmetros metabólicos dos filhos, segundo o turno de trabalho materno. A produção de melatonina das trabalhadoras noturnas parece ter sido afetada pela exposição ao trabalho noturno. Também parece que a melatonina pode ter um efeito positivo na manutenção de boas condições de saúde ao longo da gestação. Não é possível afirmar que as repercussões da exposição ao trabalho noturno, incluindo a redução de melatonina, possam apresentar efeitos no decorrer da vida dos filhos das trabalhadoras noturnas
ABSTRACT

Aim:

To investigate the patterns of melatonin production, sleep and metabolism of pregnant and non-pregnant, day and night workers. The purpose of the present study was also to analyze some of health parameters of newborns and the offspring from women workers, according to the maternal shift work.

Methods:

Two parallel studies were performed in a public hospital in the São Paulo city. In the first study, the patterns of melatonin production, sleep, anthropometry, metabolic parameters, and eating habits of day and night pregnant workers and some metabolic parameters of the newborn were investigated. The second study investigated the production patterns of melatonin, sleep, anthropometry, metabolic parameters, life and eating habits of day and night workers and some metabolic and eating habits among the offspring according to the mother's shift work.

Results:

The products of this study are described in the form of three manuscripts. The first refers to the main hypothesis of the study, in which workers who are subject to changes in melatonin levels, reduced by exposure to light at night, are more likely to develop health problems, even compromising the descendants of women workers. The second manuscript presents the study with the pregnant women, and analyzes the profile of melatonin production during the gestation, according to the work shifts, discussing additionally, the reflexes in the health of the newborns. The third manuscript describes the study carried out with mothers and children, considering the influence of exposure to light at night during gestation, and the consequent reduction of melatonin with possible repercussions on the metabolism of children who are already in their infancy or adolescence.

Conclusion:

The hypothesis presented in the first study was tested in subsequent studies. Regarding the investigation with pregnant women, presented in the second study, we highlight the reduction of melatonin production in night workers when compared to day workers, in addition to the lower Apgar score among offspring from mother's night workers. The third study, which was carried out with nonpregnant women and their offsprings, did not verify significant differences in the metabolic parameters of the offspring, according to the maternal shift work. The melatonin production of night workers seems to have been affected by exposure to night work. It also appears that melatonin may have a positive effect on maintaining good health throughout pregnancy. It is not possible to state that the repercussions of exposure to night work, including the reduction of melatonin, may have effects over the lives of the night worker's offspring
