Auditoría de los tiempos quirúrgicos en el Hospital Carlos Andrade Marín, 2015-2017 / Surgery time audit in HCAM, 2015 ­ 2017

RESUMEN

Introducción: El objetivo del estudio fue evaluar los tiempos quirúrgicos y establecer modelos de predicción para tomar decisiones que mejoren la eficiencia de los quirófanos en un hospital de tercer nivel. Materiales y Métodos: Estudio descriptivo de la actividad quirúrgica realizado en el Hospital Carlos Andrade Marín de enero 2015 a junio 2017. De 64288 registros de intervenciones quirúrgicas revisados solo 37351 fueron empleados en este estudio, luego de excluir inconsistencias errores y duplicados. ANOVA para medidas repetidas se utilizó para analizar los tiempos de cirugía, anestesia y quirófano. Un valor p<0.05 fue aceptado como significativo. Utilizamos paquetes estadísticos JASP, R y Epi-Info 7. Resultados: Los tiempos registrados mostraron distribuciones sesgadas a la derecha con diferencias significativas entre ellas. El mayor número de cirugías correspondió a Cirugía General (15.7%), Obstetricia (15%), Traumatología (15%) y Ginecología (9%). Los quirófanos más utilizadas fueron el número 6 (16.7%) empleado por varios servicios quirúrgicos, especialmente Cirugía General y el número 14 (13.5%), correspondiente a Obstetricia. Discusión: El mayor inconveniente detectado fue el registro incompleto y la falta de registro de la actividad quirúrgica en sala de operaciones. No hubo registros en casi el 30% de los pacientes intervenidos. Más del 10% de errores de registro fueron detectados. Todo lo anterior nos llevó a prescindir del 48% de los registros del banco de datos. Necesitamos mejorar el sistema de registro de las actividades quirúrgicas.

ABSTRACT

Introduction: The main goal of this study was reviewing the times employed in surgery, anesthesia and the operating room of patientes attended in a third level hospital. Methods: Descriptive study of the surgical acitiviy carried out at Carlos Andrade Marín Hospital, from January 2015 to June 2017. From 64 288 registries received only 37 351 were finally analyzed since registries containing errors and duplicates were excluded. ANOVA for repeated measures was used to analyze surgical times. A p-value <0.05 was considered significant. Statistical packages JASP, R and EpiInfo 7 were employed. Results: Time distributions were skewed to the right, therefore, the median was employed. Differences were significant among them. The largest number of surgeries belonged to General Surgery (15.7%), Obstetrics (15%), Ortopedics (15%) and Ginecology (15%). The busiest operating rooms were number 6 (16.7%) and number 14 (13.5%). Discussion: Among the problems detected, the lack of registration of surgical times reached almost 30%, plus 10% of registration errors. Therefore, we discarded around 48% of the received data. The limitations of the available data, unfortunately did not allow us to fully analyze operating room activities, therefore, we need to improve the registration of surgical activities.
