O sofrimento na perspectiva de pacientes com úlceras venosas

RESUMO

As úlceras venosas são lesões de membros inferiores causadas pela insuficiência venosa crônica e se caracterizam como um problema de saúde pública, sobretudo pela sua transcendência e vulnerabilidade. Alguns autores consideram que a convivência com uma úlcera venosa pode ocasionar mudanças e dificuldades em diversos aspectos na vida da pessoa acometida, com interferências negativas na qualidade de vida, paralelamente a sentimentos relacionados ao comprometimento da capacidade funcional e das relações sociais. No entanto, o sofrimento definido pelas próprias pessoas com úlceras venosas é pouco analisado na literatura. O objetivo do estudo foi compreender o sofrimento na perspectiva de pacientes com úlceras venosas, com enfoque em suas representações sobre o agravo. Trata-se de pesquisa qualitativa, fundamentada na Teoria das Representações Sociais, na perspectiva de Alain Giami. Foram realizadas entrevistas individuais abertas com 12 pacientes com úlceras venosas, que realizam curativos e acompanhamento em unidades básicas de saúde, no município de Belo Horizonte. Os dados foram interpretados com base na Análise Estrutural da Narração e desvelaram quatro categorias contexto de vida; impacto da existência da ferida no cotidiano; representações sobre a ferida causas, tratamento, cuidados, curativo; expectativas em relação à cura e tratamento da ferida. A análise dessas representações mostrou que o sofrimento, muitas vezes, é reflexo de uma assistência deficitária em vários aspectos, concordando com o que outros autores afirmam sobre a repercussão da assistência inadequada aos pacientes com úlceras venosas. Esta pode levar a que uma lesão permaneça anos sem cicatrizar, acarretando alto custo social e emocional. ABSTRACT

Venous ulcers are lesions of the lower limbs caused by chronic venous insufficiencyand are characterized as a public health problem. Some authors consider that livingwith a venous ulcer can cause changes and difficulties in several aspects in the life ofthe affected person, with negative interferences in the quality of life, in parallel withfeelings related to impairment of functional capacity and social relations. However, thepresence of suffering defined by the people themselves with venous ulcers is littleanalyzed in the literature. The objective of the study was to understand the sufferingfrom the perspective of patients with venous ulcers, focusing on their representationsabout the disease. This is a qualitative research, based on the Theory of Social Representations, from the perspective of Alain Giami. Individual interviews werecarried out with 12 patients with venous ulcers, who performed dressings and followup at Basic Health Units in the city of Belo Horizonte. The data were interpreted basedon the Structural Analysis of Narrative and revealed four categories Context of life;Impact of the existence of the wound in daily life; Representations about the woundcauses, treatment, care, dressing; Expectations regarding wound healing andtreatment.
