Tapa metálica de botella como cuerpo extraño en esófago / Metal bottle cap as a foreign body in the esophagus

RESUMEN

Paciente de 22 años de edad, que estando bajo estado etílico bebió una cerveza que aún tenía la tapa metálica sobre el pico de la botella, deglutiendo de forma accidental ese objeto junto con parte del contenido líquido. Presentó disfagia notoria, tos y vómito, sin lograr eliminar el cuerpo extraño. Por la persistencia del cuadro acudió a emergencia de nuestro hospital, a las 6 horas del inicio del cuadro. Al examen se encontró un paciente ansioso, aún bajo efectos de alcohol, sin disfonía y con signos vitales estables. La auscultación pulmonar y cardiaca fue normal. En las radiografías de tórax posteroanterior y cuello, se observó la presencia de un objeto metálico a nivel del cuello, compatible con tapa de botella de cerveza y localizado en tercio superior del esófago; fotos 1 y 2 (flechas). Fue valorado por Gastroenterología y en la endoscopia digestiva alta no fue posible remover el objeto por varias ocasiones. Consultaron a Cirugía Torácica, decidiendo realizar una esofagoscopia rígida y se logró retirar un cuerpo metálico, correspondiente a tapa de botella; foto 3. El paciente permaneció hospitalizado tres días en el servicio de Cirugía Torácica, bajo vigilancia de probables complicaciones secundarias a la remoción del presente cuerpo extraño, luego de lo cual fue dado de alta

ABSTRACT

Patient 22 years old, who being under ethyl state drank a beer that he still had the metal lid on the bottle's peak, accidentally swallowing that object together with part of the liquid content. He presented dysphagia notorious, cough and vomit, without managing to eliminate the body strange. Due to the persistence of the painting, to emergency of our hospital, at 6 o'clock of the beginning of the picture. Upon examination, a anxious patient, still under the influence of alcohol, without dysphonia and with stable vital signs. The Pulmonary and cardiac auscultation was normal. In radiographs of posteroanterior chest and neck, the presence of a metallic object was observed at neck level, compatible with cap beer bottle and located in upper third of the esophagus; photos 1 and 2 (arrows). It was valued by Gastroenterology and in endoscopy high digestive it was not possible to remove the object several times. Consulted Surgery Thoracic, deciding to perform an esophagoscopy rigid and it was possible to remove a metal body, corresponding to bottle cap; photo 3. The patient remained hospitalized for three days in the Thoracic Surgery service, under surveillance of probable secondary complications to the removal of the present foreign body, after which was discharged
