Treatment of unstable intra-articular distal radius fractures by ligamentotaxis

ABSTRACT
A total of 54 adult patients having 58 unstable, intra-articular distal radius fractures were treated by underwent closed reduction and ligamentotaxis using the small AO external fixator. The device was applied for six weeks, and the follow-up period ranged from six months to two years. The final evaluation was based on both the functional and the anatomical outcome. The functional [clinical] evaluation comprised deformity, pain, motion, and strength of the mist. The anatomical [radiological] evaluation included radial length, volar tilt, radial inclination, and articular congruity. The functional result was excellent in 46 cases [79.3%], good in 8 cases [13.8%], and fair in 4 cases [6.9%]. The anatomical result was nearly parallel to the functional result, with 42 cases graded excellent [72.4%], 12 cases good [20.7%], and 4 cases fair [6.9%]. The carpal height, that monitors the amount of ligamentotaxis, was almost unchanged indicating near-constant distraction throughout the period of treatment. Complication rate was low compared to previous studies, and patient satisfaction was appreciably high Ligamentotaxis, as provided by the external fixator, represents an effective and reliable method of treatment for the unstable fractures of the distal end of radius
