ABSTRACT
The treatment of pateints who are orthotropic after wearing full cycloplegic correction or who have a minimal angle of esotropia at distance and a marked non-accommodative esotropia at near fixation is considered to be difficult. In 30 patients with an esotropia at far less than +10 degrees, but with a non accommodative esotropia ranging from +10 degrees to +25 degrees we performed a modified bimedial fadenoperation with an unstretching technique during attaching the muscle to the sclera. Postsurgically 26 patients reached alignment with binocularity. In these patients the mean deviation at far was +1 degrees and at close to +2.5 degrees. Two patients had a consecutive exotropia. In two patients the operation was uneffective with remaining convergence excess. The modified fadenoperation with unstretching of the anterior segment of the attached muscle is a suitable tool for the treatment of non - accommodative convergence excess with no or small esotropia
