Single versus two layer suturing for closing the uterine incision at ceasarean section

ABSTRACT
To evaluate the impact of a single versus two-layer closure of low transverse cesarean section [LTCS] on operative and postoperative outcome, this randomized trial was done at Sohag Teaching Hospital. It included 400 women randomized into two groups, each included 200 cases Group A underwent a routine cesarean section [CS] with double layer closure of the uterine incision and closure of the visceral and parietal peritoneum and group B underwent CS with single layer closure of the uterine incision and leaving the peritoneum unsutured. The results showed a significant shorter operating time, less suture material use, less antibiotic and analgesic use in group B than group A. The follow up showed no differences in scar integrity with less adhesion formation in group B. It was concluded that single layer closure of the uterine incision and leaving the peritoneum unsutured are associated with shorter operating time, less suture material use, faster recovery and shorter hospital stay. Subsequent scar integrity and adhesion formation are also more favorable
