Reinfarto após tratamento trombolítico: resultados obtidos com a reaplicaçäo de droga trombolítica em 3 pacientes / Reinforction after Thrombolytic Treatment. Acquired Experience with Reaplication of Thrombolytic Drug in Three Patients

RESUMO

No período de 1985 a 1990, 120 pacientes com infarto agudo do miocárdio com menos de 6h de evoluçäo, foram submetidos a tratamento trombolítico. Desses, 4 pacientes, dentro de 72 h após o 1§ episódio, voltaram a apresentar os mesmos sinais clínicos e eletrocardiográficos, configurando-se assim, o reinfarto. Por ocasiäo do reinfarto, 3 dos 4 pacientes receberam a mesma dose de estreptoquinase (EQ) utilizada no 1§ episódio (1.200.000U em dois pacientes e 1.500.000U em 1 paciente), seguindo-se o mesmo protocolo de infusäo. Em 2 pacientes o infarto acometia a parede inferior e, em 1, a parede anterior. Os 3 pacientes, após nova dose de EQ, evidenciaram sinaios de reperfusäo miocárdica com a melhora da dor e do eletrocardiograma. Um paciente, 24 h após, voltou a apresentar novo quadro de reinfarto, minutos antes do estudo cinecoronariográfico; foi submetido a angioplastia de urgência, com sucesso, da coronária direita que encontrava-se totalmente ocluida. Complicaçöes: com exceçäo do paciente n§ 2 que apresentou hematoma de membro superior direito de médio porte, após tentativa de punçäo de veia subclávia para intorduçäo de eletrodo de marcapasso de urgência, os demais näo apresentaram qualquer intercorrência grave decorrente da nova aplicaçäo da droga trombolítica. No seguimento tardio de um ano pós-reinfarto, todos estavam vivos

ABSTRACT

From 1985 to 1990, 120 patients within 6h afterthe onset of acute myocardial infarction were submitted to thrombolytic treatment. In 4 patients, there were clinical and electrocardiographic (ECG) changes of reinfarction. Three of them were undergone to a second thrombolytic therapy with streptokinase (SK). Two had infarction of the inferior wall, and one on the anterior wall. The patients were managed with the same SK dosage as in the first episode and the same protocol ofinfusion. All 3 patients, after new SK infusion showed evidence of myocardial reperfusion with relief of pain and improvament of the ECG. One patient, 24h after retreatment, had another reinfarction treated with emergency angioplasty of the right coronary artery, which was totally occladed. One patient (case n° 2), presented haematoma of right arm after punction of the subclavian vein and the other two patients did not show any serious intecorrence related to the new thrombolitic treatment. All of them were found alive over one year of follow-up
