RESUMO
Stric Precautions shall be taken when administering parenteral nutrition, which is the most advisable, in the metabolic and nutritional support to patients with serious acute pancreatitis; total parenteral nutrition with an excess supply of glucose the hyperglicemia phase, results in undesirable and harmful physicologic effects; this was the reason for the proposal of administering a hypocaloric (hypoglucid) and hyperproteic regimen to a group of patients with serious acute pancreatitis, and determining its efficiency, as well as checking the behaviour of the patients and this regimen; 9 patients with serious acute pancreatitis are analysed between January 1987 and July 1992 at the intensive care unit of a private institution; those patients received a minimum of 1214 calories per day and a maximum of 1820 calories with an average of 1492, based on the harris-Benedict formula, without making increases according to their pathology. We were able to observe that in spite of having admnistered a total of 2 grs/kg/day of proteic supply, catabolism in these patients was severe, obtaining a nitrogen balance average of - 5.43 during the days in which the hypocaloric and hyperoproteic regime was administered ; mortality level of only 33.3 per cent in spite of the seriousness of the inherent pathology. It is worth noting the obesity in these patients; 100 per cent of them were well over the ideal weight, with clear overweight
