RESUMEN
Objetivo. Estimar la frecuencia de subregistro de muerte en niños en localidades rurales e identificar las características asociadas. Material y métodos. El estudio se realizó en tres etapas en la primera se formaron tres estratos de localidades según el número de habitantes; de cada estrato se eligió, de manera aleatoria, un número representativo de localidades que fueron censadas para identificar las defunciones ocurridas entre junio de 1993 y julio de 1994. En la segunda etapa se realizó autopsia verbal con las madres de los niños fallecidos para identificar la causa de muerte e investigar las variables de estudio. Por último, se identificó el registro de muerte a través de los certificados de defunción. Resultados. Se identificó una frecuencia de subregistro de 68.9 por ciento; en las localidades menores de 500 habitantes fue de 73.2 por ciento, y en las mayores, de 60.5 por ciento. Como características asociadas al subregistro se identificaron carecer de certificado de defunción y de registro de nacimiento; edad menor de un mes; desconocimiento del concepto y funciones del registro civil; distancia al registro civil y acceso al médico mayor de 30 minutos, y localidad menor de 1000 habitantes. Conclusiones. Se propone una modificación en la organización actual del procedimiento de registro de muerte, así como el establecimiento del certificado de nacimiento propuesto por el Sistema Nacional de Salud
ABSTRACT
Objective. To estimate the frequency of underregistered children deaths in rural areas and to identify associated characteristics. Material and methods. The study was conducted in three stages. In the first, three strata of communities were formed according to the number of inhabitants; then a representative number of villages was randomly selected from each stratum. A census was made in each village to determine the deaths occurred between June 1993 and July 1994. In the second stage, the verbal autopsy method was used with the mothers of the dead children to determine the cause of death and investigate the study variables. In the third stage, registration of death was verified through the death certificates. Results. An underregistration frequency of 68.9% was found; in communities of less than 500 inhabitants underregistration reached 73.2% and in larger communities it was 60.5%. Characteristics associated to underregistration were the lack of birth and death certificate, age younger than one month, lack of information on the concept and purpose of a Public Registration Office, distance from the Public Registration Office and to the doctor of more than 30 minutes, and community smaller than 1 000 inhabitants. Conclusions. A modification in the current organization of the death registration procedure is proposed and the use of a birth certificate designed by the National Health System.
