RESUMO
O objetivo do presente estudo foi avaliar a prevalência da deficiência da proteína S em pacientes com trombose venosa profunda e a interferência do anticoagulante oral durante a investigação. Foram avaliados aleatoriamente 87 pacientes, sendo 47 do sexo feminino e 40 do sexo masculino, com idades entre 17 e 56 anos e média de 36,3 anos, que apresentaram trombose venosa profunda (TVP) de membros inferiores. O diagnóstico da TVP foi feito com dúplex e confirmado pela flebografia nos casos de dúvidas. A mensuração da proteína S foi realizada pelo método coagulométrico, avaliando atividade biológica. Os exames foram colhidos um mês após a remoção da anticoagulação oral em todos os pacientes. Em trinta deles foram colhidos também durante a anticoagulação. Em 6,9 por cento dos pacientes foi confirmada, em duas mesurações, a deficiência da proteína S. Na avaliação durante a anticoagulação, os trinta pacientes apresentaram redução dos níveis dessa proteína. Concluímos que a deficiência da proteína S em nosso estudo foi semelhante ao observado na literatura, sendo que a sua investigação mais confiável deve ser realizada após a suspensão do anticoagulante oral.
ABSTRACT
The primary objective of this study was to evaluate theprevalence of Protein S deficiency in patients sufferingfrom deep venous thrombosis. As a secondary objectivethe interference of oral anticoagulation duringtreatment was studied. Forty-seven female and 40male patients with ages ranging from 17 to 56 and amean age of 36.6 years, who presented with deepvenous thrombosis of the lower limbs, were studied.Diagnosis of deep venous thrombosis was made usinga duplex doppler scan and in cases of doubt the resultswere confirmed by phlebology. Measurement of theprotein S concentrations was performed by thecoagulometric method, that is, by evaluating thebiological activity. The blood samples were collectedone month after the suspension of oral anticoagulationtreatment in all patients. In thirty patientsblood samples were also taken during anticoagulationtreatment. A total of 6.9% of the patients wereconfirmed, with two consecutive measurements, ashaving protein S deficiency. The evaluations of the30 patients during treatment all demonstrated areduction in the concentrations of this protein. Weconcluded that the prevalence of protein S deficiencyin our study was similar to the prevalence observed inother publications. We also saw that the most reliabletime to investigate protein S levels is after thesuspension of oral anticoagulant treatment.
