RESUMO
A vertigem de posicionamento (ou posicional) paroxística benigna VPPB caracteriza-se por ataques de vertigem de curta duração. Seu diagnóstico é estabelecido através de nistagmo típico desencadeado pela manobra de Dix-Hallpike e pode ser tratada de maneira eficaz com as manobras de Brandt-Daroff (MBD), de Semont (MS) ou de Epley (ME).O objetivo deste estudo, que incluiu 50 pacientes, foi comparar a eficácia da MBD, MS e ME no tratamento domiciliar da VPPB posterior e avaliar o efeito de uma segunda manobra no caso de falha terapêutica da primeira. Pôde-se concluir que após 2 semanas o tratamento domiciliar tem boa eficácia, MS e ME são semelhantes entre si e melhores que MBD, metade dos pacientes que permanecem sintomáticos após 2 semanas podem se beneficiar do tratamento por 4 semanas e resultados equivalentes são obtidos se realizadas uma ou duas manobras durante 4 semanas. /Benign paroxysmal positioning (or positional) vertigo (BPPV) is characterized by brief attacks of vertigo. It can be easily diagnosed with the Dix-Hallpike test on the basis of its typical nystagmus and effectively treated with Brandt-Daroff (BDE), Semont (SM) or Epley maneuver (EM). The aim of this study, which included 50 patients, was to compare the efficacy of SM, EM, and BDE during self-treatment of posterior BPPV and to evaluate the effect of a second maneuver if the first one failed. We conclude from our data that self-treatment of BPPV has a good efficacy after 2 weeks, SM and MEM are both equally efficient and better than BDE, half of all patients who remain symptomatic after 2 weeks, may benefit from a 4-week self-treatment and equivalent results are achieved by performing one or two maneuvers during a 4-week self-treatment...
