Contribuição da eletrocauterização laparoscópica ovariana bilateral no tratamento de mulheres inférteis com síndrome dos ovários policísticos (SOP) clomifeno-resistentes / The contribuition of laparoscopic ovarian diathermy in the treatment of women with anovulatory and clomiphene-resistant polycystic ovary syndrome

RESUMO

Foram estudadas prospectivamente 49 mulheres inférteis com síndrome dos ovários policísticos clomifeno-resistentes. Vinte e quatro mulheres foram submetidas à eletrocauterização laparoscópica ovariana bilateral e 25, a até três ciclos de indução ovulatória com hormônio folículo-estimulante recombinante. Objetivou-se: avaliar os efeitos da cirurgia sobre o volume ovariano e as dosagens séricas hormonais e comparar as taxas de gestação dos dois grupos de tratamento. Houve diminuição significativa do volume ovariano bilateral após a cirurgia. Observou-se queda significativa dos níveis séricos de testosterona e androstenediona e tendência à queda dos níveis séricos de hormônio luteinizante. A taxa de gestação obtida com a eletrocauterização ovariana, doze meses após o procedimento, não diferiu da obtida com três ciclos de indução ovulatória.Forty-nine women with anovulatory infertility and clomiphene-resistant polycystic ovary syndrome were studied prospectively. Twenty-four patients were submitted to laparoscopic ovarian diathermy and twenty-five submitted to cycles of ovarian hyperstimulation with recombinant human follicle stimulating hormone. The objectives were to: evaluate the effects of surgery on the ovarian volume and the hormone levels and compare the pregnancy rates of the two treatment groups. There was decrease of the bilateral ovarian volume after surgery. We observed fall in the levels of testosterone and androstenedione and trend of reduced to the levels of luteinizing hormone.
