Padões histopatológicos e imuno-histoquímicos do envolvimento pulmonar no sarampo: reflexos da disfunção imune / Histopathological and immunohistochemical patterns of lung involvement in measles: reflections of immune disfunction

Analisou-se fenótipos celulares (CD4,CD8,CD20,CD45RO,CD68,S100,NK) e citocinas (IL-1,IL-2r,IL-4,IL-10,IL-12,TNF,IFN) em epitélio brônquico, alvéolos e BALT de 42 necrópsias de Sarampo e 18 controles pareados com Cardiopatias congênitas. Realizou-se análises semiquantitativa (14-pneumonia de Hecht e 28-pneumonite) e quantitativa por técnica imuno-histoquímica. Houve depleção de LTCD4+, LCD20+ e células dendríticas em epitélio brônquico, alvéolo e BALT; depleção de NK e LTCD45RO+ em epitélio brônquico e BALT; depleção de células CD68+ e diminuição de IL-10 no BALT. Esses achados permitem concluir que o Sarampo ocasiona acentuada imunossupressão local pulmonar que leva a infecções respiratórias secundárias e aos altos índices de morbidade e mortalidade, especialmente em desnutridos / Cell phenotypes (CD4,CD8,CD20,CD45RO,CD68,S100,NK) and cytokines (IL-1,IL-2r,IL-4,IL-10,IL-12,TNF,IFN) were labeled in bronchial epithelium, alveoli and BALT from 42 necropsies of measles and 18 paired controls with congenital heart diseases. Were made semi quantitative (14 –Hecht’s pneumonia e 28 -pneumonitis) and quantitative analysis by immunohistochemical technique.

Results:

depletion of LTCD4+, LCD20+ and dendritic cells in bronchial epithelium, alveolus and BALT; depletion of NK and LTCD45RO+ in bronchial epithelium and BALT; depletion of CD68+ cells and reduced expression of IL-10 at BALT. These findings suggest that measles causes severe local immune suppression in lungs leading to secondary respiratory infections and high levels of morbidity/mortality, especially in malnourished...
