RESUMEN

La necesidad de preservar el tejido pulmonar en pacientes cuya capacidad respiratoria imposibilita las resecciones regladas, como lobectomía y neumonectomía, obliga a recurrir a procedimientos tolerables por estos. Se presenta una investigación prospectiva de 5 pacientes operados por cáncer de pulmón y enfermedades asociadas, a los que se les realizaron segmententomías pulmonares en el Hospital Universitario ®Manuel Fajardo¼. Los estadios de los tumores fueron I y IIA. El estudio histológico demostró que 4 pacientes padecían tumores epidermoides y 1, adenocarcinoma. Los pacientes se encontraban en la sexta y séptima décadas de la vida, con antecedentes de drogadicción por consumo de tabaco y afecciones médicas sistémicas, como diabetes mellitus (1), enfisema pulmonar o bronquitis crónica (2) y cardiopatía isquémica (2). El abordaje quirúrgico se realizó mediante incisiones axilares y entubación selectiva y se realizaron a todos los pacientes resecciones limitadas a una segmentectomía. No tuvimos mortalidad quirúrgica a pesar de que uno de los pacientes presentaba dificultades respiratorias en el postoperatorio. La supervivencia fue la siguiente 1 paciente falleció al segundo año de la operación, 4 rebasaron los tres años, 2 los cuatro años y 1, los cinco. Las resecciones económicas como la segmentectomía son una opción quirúrgica de buenos resultados en el paciente con riesgo quirúrgico alto y pruebas funcionales respiratorias deficitarias(AU)

ABSTRACT

The necessity to preserve the lung fabric in patient whose breathing capacity disables the ruled resections, as lobectomía and neumonectomía, forces to appeal to passable procedures for these. A prospective investigation of 5 patients is presented operated by lung cancer and associate illnesses, to those that were carried out lung segmententomías in the University Hospital ®Manuel Fajardo¼. The stadiums of the tumors were I and IIA. The study histológico demonstrated that 4 patients suffered tumors epidermoides and 1, adenocarcinoma. The patients were in the sixth and seventh decades of the life, with drogadicción antecedents for consumption of tobacco and systemic medical affections, as diabetes mellitus (1), lung emphysema or chronic bronchitis (2) and cardiopatía isquémica (2). The surgical boarding was carried out by means of axillary incisions and selective entubación and they were carried out to all the patient limited resections to a segmentectomía. We didn't have surgical mortality although one of the patients presented breathing difficulties in the postoperative one. The survival was the following one 1 patient died to the second year of the operation, 4 surpassed the three years, 2 the four years and 1, the five. The economic resections as the segmentectomía are a surgical option of good results in the patient with high surgical risk and deficit breathing functional tests(AU)
