Cardiografia por bioimpedância transtorácica: uma nova abordagem no manuseio de pacientes com insuficiência cardíaca / Transthoracic impedance cardiography: a new method for the management of heart failure patients

RESUMO

A cardiografia por bioimpedancia transtorácica é uma forma de pletismografia que utiliza mudanças na impedância elétrica torácica para estimar mudanças no volume sanguíneo no interior da aorta e mudanças no volume de fluído do tórax. Assim, podem-se estimar parâmetros hemodinâmicos e o status volêmico. Os trabalhos publicados com a nova geração de aparelhos mostram que esse método é útil para detectar precocemente o risco de uma descompensação e para manusear o paciente hospitalizado com insuficiência cardíaca, auxiliando na escolha da classe medicamentosa e na titulação das doses de medicações intravenosas, assim como auxiliando no prognósticio.

ABSTRACT

Transthoracic impedance cardiography is a form of plethysmography that utilizes changes in thoracic impedance to estimate changes in blood volume in the aorta and changes in fluid volume in the thorax, thus determining hemodynamic parameters and fluidstatus. The studies carried out with these new generation devices have shown that this method is useful in the early identification of patients at risk fora decompensation as well as for managing in-hospital patients with heart failure. Furthermore, they aid in the choice of pharmacotherapy options as well as the up titration of intravenous medications and function as a prognostic tool.
