Efeito da repaglinida sobre o pré-condicionamento isquêmico / The effect of repaglinide on ischemic preconditioning

RESUMO

O pré-condicionamento é conceituado como aumento da tolerância do miocárdio isquêmico aos esforços repetitivos e admitido como o resultado da abertura dos canais K-ATP dependentes. Drogas bloqueadoras destes canais induzem à perda da condição e permitem surgimentos de eventos cardiovasculares.42 pacientes diabéticos portadores de isquemia miocárdica realizaram testes ergométricos seqüenciais sem uso de medicação na fase 1 e com o uso de repaglinida na fase 2.Todos os pacientes desenvolveram isquemia nos testes 1 e 2 da fase 1.O tempo de duração do exercício para alcançar isquemia foram maiores no segundo teste (p<0,001).Com o uso da repaglinida os tempos do segundo teste foram menores que o primeiro(p<0,001). Nos pacientes diabéticos com doença coronária estável a repaglinida bloqueou o pré-condicionamento isquêmico.

ABSTRACT

The preconditioning is considered as the increase of ischemic myocardial tolerance to the repetitive efforts and admitted as the result of the opening of the K-ATP channels. Drugs that block these channels induce the loss of conditioning and allow the upcoming of cardiac events.42 diabetic patients with myocardial ischemia underwent sequencial exercise tests without medication in phase 1 and with repaglinide in phase 2. All patients developed ischemia in tests 1 and 2 of phase 1.The time of exercise to reach ischemia was greater in the second test (p<0,001).With repaglinide ,the time in the second test was smaller than in the first (p<0,001).In diabetic patients with stable coronary artery disease, repaglinide blocked the ischemic preconditioning.
