O Internato médico e suas perspectivas: estudo de caso com educadores e educandos / Medical internship and its perspectives: a case study with educators and students

RESUMO

Este trabalho teve por objetivo conhecer a percepção de educadores e educandos sobre a situação atual do Internato Médico e suas perspectivas. Foi realizado pelo método qualitativo, tipo estudo de caso, com a técnica de entrevista semi-estruturada. Foram sujeitos do estudo seis estudantes, oito professores e um médico residente engajados na discussão de educação médica. Segundo os entrevistados, o modelo de Internato Médico é, ainda, o tradicional, baseado em rodízio nas principais áreas médicas, podendo também incluir um período eletivo. Suas limitações são ênfase na especialização, predomínio de estágios no hospital, descontinuidade e falta de integração dos conhecimentos. Entre as experiências com este modelo, para superar algumas de suas limitações, foram citados o Internato Rural e as práticas junto à comunidade. Quanto ao modelo ideal, mencionou-se a necessidade de ampliar o tempo em atividades práticas, utilizando metodologias ativas de ensino-aprendizagem, e de inserir os estudantes no sistema de saúde desde o início da formação, com responsabilidade crescente, nos diversos níveis de atenção, até a extinção do nome Internato. Conclui-se que ainda há um caminho a percorrer para alcançar o modelo almejado, sendo necessário maior diálogo entre os sistemas de educação superior e de saúde.

ABSTRACT

The aim of this work was to learn how educators and students perceive the current situation of medical internship and its perspectives. It was carried out using the qualitative method, type case study, with semi-structured interviews. The subjects of the study were 6 students, 8 professors and one medical resident engaged in the discussion on medical education. According to the interviewees, the medical internship model is still the traditional one, based on turns in the main medical areas. The limitations of this system are the emphasis given to specialization, the predominance of training periods in the hospital and the discontinuity and lack of integrated knowledge. Experiences mentioned as capable of helping to overcome some of the limitations of the model were the Rural Internship and community-based practices. As to the ideal model one identified the need of more time spent in practical activities, use of active teaching-learning methodologies and insertion of the students since the beginning of the course in the health system, in the different levels of attention and with increasing responsibility, until extinction of the name Internship. It is concluded that there is still a long way to go for reaching the ideal model, including the need for more dialogue between the higher education and health systems.
