Papulosis linfomatoide asociada a leucodistrofia cerebral / Lymphomatoid papulosis associated with cerebral leukodystrophy

RESUMEN

La papulosis linfomatoide (PL) es una rara condición linfoproliferativa cutánea, a veces autolimitada caracterizada por una erupción crónica y recurrente de pápulas y nódulos que tienen regresión espontánea, la cual se enmarca en el espectro del linfoma cutáneo de células T CD30(+). Presentamos el caso de paciente de sexo masculino, de 27 años. portador de PL y leucodistrofia cerebral asintomática. Focalizamos la discusión en el diagnóstico histológico con sus subgrupos A-B-C, y en una actualización de la terapéutica empleada. Hasta el momento el tratamiento más eficiente es el metotrexato oral en dosis bajas, que logra remisiones importantes, aun cuando haya recidivas. La asociación con leucodistrofia cerebral nos parece meramente incidental.

ABSTRACT

Lymphomatoid papulosis (LyP) is rare, sometimes self-limiting, cutaneous lymphoproliferative condition characterized by a chronic recurrent eruption of papules and nodules that undergo spontaneous regression, which exists on a spectrum of cutaneous T CD30(+) large cell Lymphoma. We present the case of a 27-year-old male patient with lymphomatoid papulosis and asymptomatic leukodystrophy. We focus on the histologic diagnosis, with is three subgroups A-B-C, and update therapeutic options. Until now the most efficient treatment is low dose oral methotrexate, which achieves complete resolution of lesions but is not free recurrences. We conclude that the association with hypomyelinating leukodystrophy is absolutely incidental.
