RESUMEN
La supervivencia de los recién nacidos extremadamenteprematuros aumentó notablemente en losúltimos años debido a la mejoría del cuidadoperinatal, en especial, mayor utilización de corticoidesprenatales, adecuada recepción en sala de partos,optimización de los cuidados iniciales, utilizaciónde surfactante y mejores técnicas de asistenciarespiratoria. Sin embargo, este aumento de la supervivenciano se asoció con una disminución desimilar magnitud en la morbilidad. Por el contrario,algunos trastornos, como displasia broncopulmonar,sepsis, dificultades en el crecimiento y complicacionesneurológicas futuras podrían estar incrementadas.El propósito de este artículo es analizar,desde la bioética, distintos aspectos del cuidadoclínico perinatal en los límites de la viabilidad
ABSTRACT
The survival rate of extremely preterm infants improved over the last years as a result of a better prenatal and neonatal care mainly due to a greater use of antenatal steroids, appropriate managementin the delivery room and in the initial care, surfactant therapy, and better modalities of assisted ventilation. However, this improvement in survival has not been associated with an equal reduction in morbidity. In fact, the frequency of bronchopulmonary displasia, sepsis, poor growth, and neurological disorders in the future may have increased. The purpose of this article is to examine, from a bioethical point of view, different aspects in perinatal care at the threshold of viability.
